ELECTRIC PULP TESTING

THIS TEST IS USED TO DETERMINE PULP BY NERVE STIMULATION


ie it STIMULATES Aδ SENSORY FIBRES .
THE OBJECTIVE IS TO STIMULATE A PULPAL RESPONSE BY
SUBJECTING THE TOOTH TO ELECTRIC CURRENT .A POSITIVE
RESPONSE IS AN INDICATION OF VITALITY AND NO RESPONSE TO
ELECTRIC STIMULUS CAN BE INDICATION OF PULP NECROSIs
 Positive response to vital fibers present
 Fails to provide information of vascular supply
 False Positive response
 False Negative response
TECHNIQUE

 Describe the test to the patient in a way that will reduce anxiety and will
eliminate a biased response
 Isolate the area of teeth to be tested with cotton rolls and saliva ejector and air-
dry all the teeth
 Check the electric pulp tester for function and determine that current is passing
through electrode
 Apply an electrolyte on the tooth electrode and place it against the dried enamel
of the crowns occlussabuccal or incisalabial surface .It is important to avoid
contacting any restoration in the tooth or the adjacent gingival tissue with
electrolyte or the electrode,this would cause a false response.
 Electric current must be complete a circuit from the electrode through the
tooth to the body of the patient then back to the electrode. When gloves
are not used the circuit is completed by dentists finger with electrode .
 When using rubber gloves one of the 2 methods must be followed-:
 A ground attachment may be clipped onto the patient’s lip
 The patient may complete the circuit by placing a finger on the metal
electrode handle .The second gives a advantage of giving patient more
control as the patient can simply lift the finger off the electrode handle
when sensation is felt .This will immediately interrupt the current and
terminate the stimulation.
 First the test is performed on vital tooth so as know the normal
stimulus as it differs from person to person a

Placement of the tester is critical to ensure accurate response from the


tooth.The most advisable areas for placing the tester is that the incisal
third for the anterior teeth and the mid-third of posterior teeth.
Retract the patient’s check away from the tooth electrode
Turn rheostat slowly to introduce minimal current into the tooth and increase the
current slowly. Ask the patient to indicate when sensation occurs by using such
words as “Tingling” or “warmth”.
FACTORS AFFECTING THE TEST
False Positive Response -:
Electrode/Electrolyte contacts the gingiva, allowing the current to reach the
attachment apparatus.
Failure to isolate and dry the teeth properly
Anxious patient :A hyperactive or neurotic patient may raise his hand as soon as
the electrode touches the tooth
A false positive can occur when moist necrotic pulp is present in the root canal
because the gases collected inside the canal may conduct current to the
attachment apparatus
In multirooted teeth in which the pulp is partially necrosed and nerve fibers are
still vital in one or more of the root canal.
False Negative Response-:

Patient heavily pre medicated with analgesic , Sedative, narcotic, alcohol.


Patient Teeth with extensive restoration and pulp protecting base
Recently traumatized teeth because presence of inflammatory exudate or
severance of nerve supply at apical foramen.
Recently erupted teeth with incomplete root formation .Because these tooth
have have under developed plexus of Rashkow as well as greater unmyelinated
nerve fibers
with an usually high threshold If calcification in the pulp and dentine is extensive
as dentine is poor conductor
More current is needed to elicit a response in a tooth with increased reparative
dentine and diminishing pulp cavity or in fibrotic pulp
