NARRATIVE REPORT FOR READING OF FORMS

FOR SECOND QUARTER

RATIONALE

The Grade 8 Curriculum headed by Mrs. Rauda I. Abdurasid


had their reading of school forms 137,38 and others. The said reading of
forms took place at the Faculty Room. Grade 8 teachers flocked the
faculty room bringing with them their school forms including student
cards, forms 137 and the like. The curriculum chair was so busy that she
took her lunch later but she was, in a sense, satisfied that the bunch of
teachers under her supervision were done with that important activity.
The Reading of forms is done every grading period as a way of
ensuring school forms were free from errors. There is a need for a group
of teachers to look into the documents and check these as to accuracy to
make sure the data are harmonious with other forms. In this way, school
forms will be thoroughly evaluated and checked to avoid mistakes or
errors in data Sinput.

OBJECTIVES

The Reading of school forms aims to:


 Ensure school forms are free from errors, defects etc.
 Organize teachers to form an evaluation team for this purpose
 Empower the curriculum chair to supervise teachers under his/her
prescribed department
 Serve as a venue for teachers to work together as a team

READING OF SCHOOL FORMS (3RD QUARTER)

The Reading of school forms for the Grade 8 curriculum took


place at the faculty room last November 29, 2019. Grade 8 teachers were
able to manage their time to finish this important activity since Dr. Alejo
had already assigned the dates for the different year levels/grade
curriculum.
The Grade 8 Curriculum Chair, Mrs. Rauda Abdurasid, spent
hours to evaluate and to check the school forms of teachers as to their
accuracy. She also took time to write down her comments during the
reading process. She also provided useful advices to teachers with
problems regarding school forms. She also patiently helped the teachers
in filling information to forms lacking needed data.
The reading of school forms started at 1:00 PM until 2:00 PM.
Teachers went away from the faculty room satisfied although they were
tired and hungry because they took their lunch later.
