Conditions for Use

i) Form F/F1 is not transferable to another PSP for use


ii) Purchased Forms are not refundable
iii) If any unused form is discovered to be missing, it is the duty of the
PSP to inform BEM in writing as soon as reasonable upon discovery.
iv) PSP to deposit Form F/F1 and Form Gs to Local Authority and Board
of Engineers Malaysia within 14 days from the issuance of Form F/F1
v) Refer to www.bem.org.my on guide to fill in Form F/F1 and Form Gs
