NON-CARBAPENEM THERAPY AS AN ALTERNATIVE FOR CHILDHOOD

URINARY TRACT INFECTION DUE TO EXTENDED-SPECTRUM Β-LACTAMASE


(ESBL) PRODUCING ESCHERICHIA COLI IN BPJS ERA : A Case Report

A 4-year-old children came to hospital with the main complaint of high fever since 10
days before. The complaint was also followed by pain during urinating. It felt every time patient
going for pee and the frequency of urination is increase. Complaints of pain during urination is
often experienced by the patients and this complaint is the worst. Other patient complaints are
nausea and decreased appetite. No dispneu or seizure.

On physical examination, height 103 cm (TB / U <P (?)) and weight 14.5 kg (BW / U <P
(?)). Nutritional status is ……… (Waterlow criteria (?)%). The vital signs include heart rate is
108 times per minute, respiration rate is 27 times per minute, axillary temperature 38.5 Celsius.
Patient felt pain when palpated on supra public area, there is no CVA pain, and no mass is
obtained. Physical examination of the genitalia showed the proximal urethral is closed by the
glans penis foreskin and cannot be pulled back.

Laboratory finding: hemoglobin 13.2 g / dL, leukocytes 23,390 / mm3, platelets 299,000 /
mm3. Urinalysis: urine is yellow, slightly cloudy, full leukocytes, and negative erythrocytes,
ketones +3 and Ph 6.0. The patient performed a urine culture, and the results are : organism
found was Escherichia coli which produced ESBL (+). From the results of the culture test, there
are several antibiotic are sensitive to treat the infections, one of them is gentamicin.

From anamnesis, physical examination and laboratory testing, the patient is diagnosed
with urinary tract infection due to ESBL infection and phymosis. Patients receive gentamicin 35
mg / 12 hours / IV (for 7 days). And the other medication for the patient are intra vena fluid
drugs D5 ¼ NS 18 drop per minute, Ampicillin 400 mg / 6 hours / IV (for 3 days), Paracetamol
150 mg / 8 hours / IV (if tax ≥ 38.5˚C), Ranitidine 15 mg / 12 hours / IV, Ibuprofen syr 5 cc / 6
hours / per-oral and Domperidone syr 5 cc / 8 hours / per m-oral (if nausea /vomit). Fluid
requirements: 1225 cc / 24 hours. Patients treated for 7 days, patients showed getting better from
day to day. The patient is completely recovered in day 7 th and prepared for circumcision for the
next 3 days by policlinic of surgery.
