    2. Respectfully submitted hereunder is the Weekly after Activity Report


    undertaken by Mexico Municipal Police Station Sandinig CL 2019-02 Field Training
    Program/On the Job Training (FTP/OJT) under PMSg Oliver Fulguirinas, Field Training
    Officer (FTO) with direct supervision of PLTCOL Juritz B Rara, Chief of Police of Mexico
    Municipal Police Station

    Summary of Activities:

    a) On March 30, 2020 at 8:10 AM, Field Training Officer (FTO) PMSg Oliver F
    Fulguirinas conducted a Daily Personnel Accounting Report (DPAR) of Mexico
    Municipal Police Station OJT/FTP detailed at Brgy. San Jose Molino, Mexico,
    Pampanga.
    b) On same date after the DPAR post at the controlled points with the presence of
    PMSg Oliver F Fulguirinas to check if they have quarantine pass and also to
    check the temperature of those person using thermal scanner.
    c) On the same date at around 8:55 PM conducted an accounting with the presence
    of FTO PMSg Oliver F Fulguirinas at front of San Jose Malino Barangay hall

    d) On March 31, 2020 at around 8:01 AM, DPAR of Mexico MPS OJT/FTP at San
    Jose Malino, Mexico Pampanga
    e) On On same date after the DPAR continue to post at controlled points at San
    Jose Malino, Mexico, Pampanga.
    f) On the same date at around 8:48 PM conducted an accounting with the presence
    of FTO PMSg Oliver F Fulguirinas at front of San Jose Malino Barangay hall.

    g) On April 1, 2020 at 8:02 AM Daily Personnel Accounting Report (DPAR) of Mexico


    Municipal Police Station was conducted by the Field Training Officer, PMSg Oliver F
    Fulguirinas at the San Jose Molino barangay hall.
    h) On same date, FSTP/OJT of Mexico MPS continuation of their duty at Controlled
    points of Brgy. San Jose Malino, Mexico Pampanga.
    i) On the same date at around 7:58 PM conducted an accounting with the presence
    of FTO PMSg Oliver F Fulguirinas at San Jose Malino Barangay hall.

    j) On April 2, 2020 at 8:08 AM Daily Personnel Accounting Report (DPAR) of


    Mexico Municipal Police Station OJT/FSTP with the presence of Field Training
    Officer, PMSg Oliver F Fulguirinas at the San Jose Molino barangay hall.
    k) On the same date at around 3:00 PM we went to the Pampanga Police Provincial
    Office for the rehearsal of the tribute dance for frontliner that will be presented at
    Jose B. Lingad Memorial Hospital, City of San Fernando Pampanga.
    l) On the same date at around 8:59 PM conducted an accounting with the presence
    of FTO PMSg Oliver F Fulguirinas at San Jose Malino Barangay hall.

    m) On April 3, 2020 we went to Jose B. Lingad Memorial Hospital for the tribute
    dance for the frontliners.
    n.) On the same date at around 9:25 AM after the dance performance at JBL Memorial
    Hospital City of San Fernando, Pampanga. conducted an accounting with the presence
    of FTO PMSg Oliver F Fulguirinas.
    o.) On the same date at around 8:48 PM conducted an accounting with the presence of
    FTO PMSg Oliver F Fulguirinas at San Jose Malino Barangay hall.

    p.) On April 4, 2020 at 8:57 AM Daily Personnel Accounting Report (DPAR) of Mexico
    Municipal Police Station was conducted by the Field Training Officer, PMSg Oliver F
    Fulguirinas at the San Jose Molino barangay hall.
    q. On the same date FTO PMSg Oliver F Fulguirinas, conducted a rank inspection of
    the team members of PSFTP CL SANDINIG 19-02 (San Jose Malino, Mexico,
    Pampanga).
    r.) On the same date at around 9:14 PM conducted an accounting at San Jose Malino
    Barangay hall.
