Informed Consent

This document provides informed consent information for a research study being conducted by students at Leyte National High School. The purpose of the study is to understand students' perceptions of pre-marital sex based on their lived experiences. Good day!
You are invited to participate in this qualitative research study being conducted by Grade
11 Science, Technology, Engineering and Mathematics (STEM) students in partial
fulfillment of the requirements for Practical Research I subject at Leyte National High
School (LNHS).

The purpose of this study is to identify and to understand as to how pre-marital sex
affects students based on their lived-experiences.
With your approval, you will be asked to participate in a face to face interview with the
researchers using open-ended questions at approximately 15 to 30 minutes regarding our
study. Rest assured that you will be provided with the interview-transcript and will be
asked to check for accuracy. The full interview will be recorded on a digital voice
recorder/cellphone however your identity will not be revealed in the final manuscript.
Though there will be no direct benefit to you for your participation in this study, we hope
that the information obtained from this study may not only contribute to the body of
knowledge but of great help in capturing an in-depth understanding of the phenomenon
being studied. Hence your participation on this research is very important and highly
appreciated.

We assure you that your responses during the interview will be treated with utmost
anonymity and will be securely kept to preserve confidentiality. Pseudonyms, code
names and other coding techniques will be immediately applied to replace personal
identifiers.
