TRANSACT ANY OFFICIAL BUSINESS AT THE DEVELOPMENT BANK OF THE PHILIPPINES (DBP)
TACURONG BRANCH, TACURONG CITY FOR AND IN BEHALF OF THE BARANGAY GOVERNMENT
OF LAGUINDING, ESPERANZA, SULTAN KUDARAT.”
WHEREAS, considering the importance of the authorization of the aforementioned newly

elected Punong Barangay and the newly appointed Barangay Treasurer and upon thorough
deliberation, the Sangguniang Barangay unanimously granted the urgent necessity of the primary
intention of the above-quoted resolution for the benefit of the sanggunian and likewise for the
entire development of the barangay;
THEREFORE, premises considered, the august body moved to a joint sponsorship, be it;
RESOLVED AS IT IS HEREBY RESOLVED to authorize the newly elected Punong Barangay,

MAXIMINO S. SULLANO and the newly appointed Barangay Treasurer, JOSIELYN F. LAGDAMEN
to transact any official business at the Development Bank of the Philippines (DBP), Tacurong
Branch, Tacurong City for and in behalf of the Barangay Government of Laguinding, Esperanza,
Sultan Kudarat;

of the majority of the sanggunian as provided in Section 394(a ) and Section 395(a) respectively
of the Republic Act 7160, otherwise known as the Local Government Code 0f 1991;
WHEREAS, the aforementioned newly appointed Barangay Secretary and Barangay

Treasurer are of legal age, qualified voters and they are actual residence of the barangay
concerned;
WHEREAS, both of them are not members of the sangguniang barangay, government
employees and not relative of the punong barangay within the fourth civil degree of
consanguinity or affinity.
NOW THEREFORE, on foregoing premises, the august body moved to a joint sponsorship,

WHEREAS, the acquisition of loan from the aforementioned lending institution is very
much necessary to augment for the proposed plans and projects of the Sangguniang Barangay
members;
THEREFORE, premises considered upon motion of Kag. Edgar B. Aguirre and duly seconed
by Kag. Merlinda B. Pendaliday, be it;

WHEREAS, presented in session for deliberation by the august body was the request of
NIGEL P. SUBIDO for a resolution regarding the conversion of 10,000 sq.m. which is a portion of
the parcel of land area registered under his name from agricultural to commercial area covered
by Cadastral Lot No. 908-B, PSD-12-000953, Assessor’s Lot. No. 065 with Certificate of Title No.
T-02024 located at Laguinding, Esperanza, Sultan Kudarat.
WHEREAS, the said conversion is intended for his future utilization of the converted area
into commercial business establishment and other related commercial projects;
WHEREFORE, considering the foregoing premises, upon motion of Kag. Leila T. Lagdamen
and duly seconded by Kag. Reynaldy C. Ladia, be it;
RESOLVED AS IT IS HEREBY RESOLVED to grant the request of NIGEL P. SUBIDO for the
conversion of his 10,000 sq.m. land area from agricultural into commercial area being described
and specified on the above-quoted resolution;

Chairman to sign a Memorandum of Agreement (MOA) as partner of the Department of
Agriculture in project implementation;
WHEREAS, considering the importance of the accreditation, it is imperative that the

august body shall appropriately comply the required documents to access for funding assistance;
NOW THEREFORE, premises considered and upon motion of Kagawad Leila T. Lagdamen,
and duly seconded by Kagawad Reynaldy C. Ladia, be it;
RESOLVED AS IT IS HEREBY RESOLVED , that the Barangay Local Government Unit (BLGU)
of Laguinding, Esperanza, Sultan Kudarat will apply for accreditation to the Department of

Ordinance No. 058 – 14th Sangguniang Bayan, Series of 2018 relative to the implementation of
Zero Open Defecation (ZOD) under the program of the Department of Health (DOH) which will
be given attention for the welfare of the entire constituents of the municipality;
WHEREAS, the barangay government of Laguinding confirm its intention to fully support
the said important endeavour objectively to promote sanitary hygiene among its constituents in
order to eradicate unsanitary practices of the inhabitants therein;
WHEREAS, the effective implementation of the program, has a great impact for the health
security of the entire barangay constituents.
WHEREFORE, considering the necessity of the program, the august body unanimously
moved to a joint sponsorship, be ti;

WHEREAS, one of the primary responsibilities of local government unit

is to provide a health system that is equitable and responsive to the health
needs of the Constituents.

Section II. Duties and Functions

1. Prepare Annual Health plans, in accordance with standard criteria and
priority programs set by the Municipal Health office.
2. Allocate Budget for the Implementation of the health Programs and
maintenance of the Health facilities services within the locality
3. Service as advisory committee to the municipality of Local Health
Board on Health matters such as area based program planning for health,
community health and organization maternal and infant death review, among
others
Specifically, the members shall have the following roles and

responsibilities;
1. Barangay Chairman (Chairman of the Board)

a. Facilitate monthly meeting
b. Overseer of the Barangay Health status and the 6 building
blocks of the health system
c. Accountable to the health plans made
d. Enact regulations and policies for sustainability of the health
system
e. Build or strength linkages with key stakeholders in health
f. Coach & mentor Barangay Council.
2. MIDWIFE(Vice-Chairman of the Board)

a. Co-Facilitate monthly Meetings
b. Overseer of the Barangay Health status and the delivery of the
health services and programs.
c. Prepare appropriate budget plan for health program & activities.
d. Consolidate and assess health reports from the barangay and
other stakeholders.
e. Update the board on health issues and programs.
3. Kagawad on Health
a. Influence/link with the Barangay Council on Budget allocation and
enactment of Barangay policies and Ordinances
b. Ensure community participation in the planning for Barangay
health policies and Ordinances.
4. BARANGAY TREASURER
Install and maintain an internal audit system of health expenditures;
Prepare and submit health financial statement to the Barangay

health board;
Appraise the Barangay health board on the financial condition and

operation of the health services;
Certify to the availability of budgetary allotment to which expenditures
and obligations may be properly charged;
Review supporting documents before preparation of vouchers to
determine completeness of requirements;
Record and post in index cards details of purchased furniture,
fixtures and equipment including disposal thereof, if any;
Account for all issued requests for obligations and maintain to keep
all records and reports related thereto;
Exercise such other powers and perform such other duties and
functions as may be provided by law or ordinance.
Advise the Barangay health board members regarding disposition of

Barangay government such other matters relative to public finance;
Take custody of the funds of the Barangay health board and health
office;
5. KAGAWAD ON PEACE AND ORDER

a. Directs, coordinates and controls the operation of the uniformed
and non-uniformed personnel to assist in the health activities.
b. Study and recommends changes or innovations in the organization,
administration and operation of the health program to make it more
responsive to the needs of the community.
c. Establishes a comprehensive and understandable system of
direction which include policy statements, general and special
orders, training bulletins, standard operation procedures, and rules
and regulations.
d. Develops an effective public relations programto promote and
maintain public understanding, confidence and support.
e. Coordinates with the Barangay Health Station in the
implementation of Community Based Rehabilitation program.
6. WOMEN’S REPRESENTATIVE
a. Ensure the participation and inclusion of women in health program
planning and implementation.
b. Recommends innovations, activities for the benefits of women.
c. Participates in all the programs of the health office.
7. INDIGENOUS PEOPLE REPRESENTATIVE

a. Represents the indigenous people in planning and implementation
of health programs.
b. Recommends programs that are beneficial for IPs that promotes
better health outcomes.
c. Develops action plan to increase health seeking behavior of IPs
thereby increasing their participation and involvement.
8. BHW PRESIDENT
a. Disseminate with other BHWs information about health programs.
b. Coordinate with BHS and BHS staff and Brgy. Captains regarding
roles in health programs.
c. Provide assistance in the implementation of health programs in
their respected barangay.
9. SCHOOL PRINCIPAL
a. Partners in health information dissemination and education
b. Conducts monitoring and evaluation on implemented health
programs.
c. Collaborates with other stakeholders for strengthening health
programs.

WHEREAS, it is the policy of the state to protect and prioritize the need of the under
prevelege sick, elderly, disable, women and children and promote the right to health and still
health conscious among them;
WHEREAS, Department Order No. 2011-0188 on Kalusugan Pangkalahatan Execution Plan

and implementation Arrangements specifies the deployment of CHTS to perform both navigation
and basic service delivery functions to targeted families;
WHEREAS, the mobilization of community Health Teams (CHTS) is a strategy of DOH to

guarantee that every family in the community is regularly visited refer to health provider to
ensure targets and objectives of the Millennium Development Goals and the Kalusugan
Pangkalahatan thrust are met;
WHEREAS, this strategy has a special focus on families identified under the National
Household Targeting System (NHTS) of DSWD in delivering key health message and is navigating
the household to access health care services on the area or localities;
WHEREAS, each Barangay is composed of management core group and its members, the
community volunteers and would typically be chaired by the Barangay Chairman and Co- chaired
by the Rural Health Midwives together with their members SB on Health and Nutrition Council
and representatives of the BHW’s.
WHEREFORE, premises considered, be it resolved, as it is hereby resolved, to support the

organization and mobilization of Community Health Teams (CHTS) composed of five community
volunteers per 100 NHTS- PR families 1. Marife L. Alcones 2. Leda Lagsub 3. Leonila Magallanes
4. Celestina Piloto 5. Liberty Lasanas at Barangay Laguinding are trusted with giving direct
assistance to underserved family specially those identified by DSWD in NHTS, including those

WHEREAS, Section 21 of the Republic Act 10121 and its implementing Rules and
Regulations provide for the allocation of 705 of the 5% calamity fund for preparedness and
mitigation purposes and the 30% shall serve as the quick response fund which can only be
released upon the declaration of state of calamity by the Sangguniang members with the
recommendation of the Barangay Development Council of the Barangay.
WHEREAS, the execution of the allotted budget for disaster preparedness and quick
response fund is authorized with enactment of the appropriation ordinance providing funds for
the programs, projects and activities embodied in the disaster risk reduction management plan
in responding to the occurrence of any natural and manmade calamity in the locality;

gadgets/Icom as its component since the previous equipment/facilities are already unserviceable
wherein our barangay officials and tanods cannot quickly response for whatever irregularities
may happen in the barangay and likewise to immediately report to the police for reinforcement;
WHEREAS, the barangay government has limited resources to augment the provision of
the gadgets, thus the proper intervention is to ask assistance to generous officials in order to

WHEREAS, presented in the joint session for consideration and appropriate action was
the immediate provision of handheld radios to be utilized by the barangay peacekeepers
and responders (barangay officials/tanods) of the barangay whenever there will be occurrence
of cattle rustling, illegal drug activities and other criminalities as the case maybe;
WHEREAS, the barangay had already installed a base, unfortunately there are no available
gadgets/Icom as its component since the previous equipment/facilities are already unserviceable
wherein our barangay officials and tanods cannot quickly response for whatever irregularities
may happen in the barangay and likewise to immediately report to the police for reinforcement;
WHEREAS, the barangay government has limited resources to augment the provision of
the gadgets, thus the proper intervention is to ask assistance to generous officials in order to
realize this very vital endeavour;
NOW THEREFORE, considering the relevance of the provision of the gadgets, on motion
of Mr. Elmer Ubaldo, purok president of Lower Sariwa and duly seconded by Mr. Raphy A. Arenga,
purok president of Purok Masagana, be it;
RESOLVED AS IT IS HEREBY RESOLVED to request Hon. Joseph Ortiz to allocate funds in

the amount of One Hundred Thousand Pesos (P 100,000.00) intended for the provision of ten
(10) units handheld radio;

assigned personnel who will serve as security guard of the pupils going in and out of the school
at the national highway;
WHEREAS, the members of the barangay council had committed a voluntary financial
assistance in the amount of One Thousand One Hundred Pesos ( P 1,100.00 ) per month to
augment for the incentive granted by the school to the assigned security personnel for the safety
of the pupils during school days for the entire school year;

WHEREAS, Section 15 of Republic Act No. 7160, otherwise known as the Local
Government Code of 1991 provides that every local government unit created or reorganized
under this code is a body politic and corporate endowed with powers to be exercised by it in
conformity with law;
WHEREAS, relative to this to this code, the Sangguniang Barangay made its final decision
to file a petition to RAFFY PEÑARANDA, proprietor of RAFFY FUNERAL SERVICES located at Purok
Mabuhay, Laguinding, Esperanza, Sultan Kudarat to stop his business operation and likewise to
strongly recommend for the cancellation of his business permit as funeral service provider for
showing disrespect and disobedience to the barangay government;
WHEREAS, the most grievous acts done by the respondent covered in the Katarungang
Pambarangay Law were non-appearance of three (3) consecutive summons served to him and
considering the barangay government a small entity that is incapable to mediate or settle any
kind of dispute maybe occurred in the barangay;
WHEREAS, in addition, he showed misconduct to our messenger who served summons
to him by insulting and provoking anger and the worst thing he said to our messenger was that
the punong barangay who sent and signed the summons were not appropriate action taken,
hence he will be put to shame of it;
WHEREAS, with this disrespectful actuation of the respondent, it is imperative that the
barangay council will make an appropriate disciplinary action to him in order not to happen the
same;
