Data Privacy Consent Form

You are invited to participate in a room-based examination for the study “The Relationship

Between the Technological Factors and The Basic Literacy Skills Among the Grade 12 Senior High

School HUMSS Students of De La Salle University – Dasmarinas” . This study is being conducted

by a research group from STEM-26 of De La Salle Dasmarinas Senior Highschool. The duration

of the examination would depend upon the time it would take for you to answer the questions given.

PARTICIPATION

Your participation in this examination is voluntary. You may refuse to take part in the

research or exit the examination at any time without penalty. If you agree to participate in this

examination, you are required to answer all the given question to prevent any lack of data during

the process of conducting this study.

BENEFITS

You will receive no direct benefits from participating in this research study. However, your

contribution would greatly support the completion of this study.

RISKS

There are no risks entwined in participating in this examination. None of the

participants would experience any form of emotional or physical abuse coming from the

research group. This examination is for research purposes only and has no hidden agenda.

CONFIDENTIALITY

All forms of data collected from this examination will only be used for the completion

of the said study. All data will be under strict confidentiality. Only those involved in the

process of completing the study (research group, research adviser, research teacher,

statistician) are allowed to access the data collected from this examination.
