    AFFIDAVIT

    I, Jose Sixto Raphael "Dingdong" Gonzalez Dantes III, of legal age,


    married to Marian Rivera Gracia-Dantes, and a resident of 16 Spencer Street,
    Cubao, Quezon City, 1111 Metro Manila, under oath depose and say that:

    1. I am the owner of one Dildolux Supreme LTD which was bought last
    January 1, 2020 at Ding Dong Cumest located at 69 Candida Street, Ermita
    Manila;
    2. I opened the package on or about 1:00 P.M of the same day, and noticed that
    the item was damaged to the extent that it could not be properly used;
    3. I drove back to the said store on or about 3:00 P.M of the same day to have
    the purchased item replaced;
    4. I was two blocks away from my point of destination when an auto rickshaw,
    or “kuliglig,” crashed into my car’s left tail light and bumper;
    5. I alighted from my vehicle to check my car and the damages;
    6. I came back to my car and checked the item I purchased from the said store
    but to no avail;
    7. I diligently searched for the Dildolux Supreme LTD in my car but I could no
    longer locate it and considered it therefore beyond recovery; and
    8. I have executed this affidavit to certify the foregoing for whatever legal
    purpose this may serve.

    Affiant further sayeth naught.
