PETITION

PETITIONER, unto this Honorable Court, respectfully avers:

1. Petitioner is a Filipino Citizen with residence at


__________________________. She has been a resident of the Philippines
since birth.

2. Petitioner is __ years old having been born on _____________


as evidenced by her Certificate of Live Birth, a copy is hereto attached and
made a part hereof as Annex “A”.

3. Petitioner graduated with a degree of Bachelor of Laws at the


___________________ in ____. Petitioner took the ____ Bar Examinations
and passed the same.

4. Petitioner is an attorney-at-law and a member of the Philippine


Bar, having been admitted as such on ____________, with Roll of Attorneys
No. _______ as evidenced by a certification issued by the Office of the Bar
Confidant. A copy of the said certification is hereto attached and made a part
hereof as Annex “B”.

5. Petitioner is currently a member in good standing of the


Integrated Bar of the Philippines (IBP) as evidenced by the certification issued
by two (2) executive officers of the IBP _______Chapter. A copy of the said
certification is hereto attached and made a part hereof as Annexes “C” and
“C-1”.

6. Petitioner has no pending administrative case against him before


the Office of the Bar Confidant. A copy of the corresponding certification
issued by the said office is hereto attached and made a part hereof as Annex
“D”.

7. Likewise, petitioner has no pending criminal case involving


moral turpitude as evidenced by the certifications issued by the Office of the
Clerk of Court of ______ City. Copies of the said certification is hereto
attached and made an integral part hereof as Annexes “E” and “E-1”.

8. Petitioner is one of the admitted members of the Bar and already


paid her membership fee for _____ as evidenced by IBP Official Receipt No.
_______ dated ____________, a copy of which is hereto attached and made
a part hereof as Annex “F”. In addition, he paid his professional tax for the
fiscal year _______ as evidenced by Receipt No. ______________ dated
________________, a copy of which is hereto attached and made a part hereof
as Annex “G”.

9. Petitioner affixes her specimen and customary signature in a


clean sheet of paper hereto attached as Annex “H”.

10. In addition, three (3) 2x2-sized photographs of the petitioner with


her signature affixed thereon are hereto attached and made parts hereof as
Annexes “I”, “I-1” and “I-2”, respectively.

11. Petitioner has purchased a notarial seal. The imprints of the said
seal are likewise hereto attached and made part hereof as Annex “J”.

12. Petitioner possesses all the qualifications and none of the


disqualifications for the office of the Notary Public.

13. Pursuant to the New Notarial Law, Petitioner additionally states


that:

compliance period pursuant to Section 3(a) of Bar Matter 850, as
supplemented by MCLE Governing Board Order No. 1, series of 2008 and
OCA Circular No. 67-2008.

15. Petitioner undertakes to submit her monthly reports to the


appropriate offices in compliance with the 2004 Rules on Notarial Practice.
