    Disciplinary Action Policy – GEO-003


    1.1 Disciplinary Action Policy and commitment of Accountable Manager

    Safety is and should always be the highest priority in all our business activities. To ensure that
    commitment, it is imperative that we take appropriate and responsible action following any incidents
    and occurrences that compromise the safety of our operations. Generally, at (insert company name)
    no disciplinary action will be taken against any employee following their participation in an error
    investigation in accordance with our “Reporting Policy”, including those individuals who may have
    breached our operating procedures. Disciplinary action will be limited to the following circumstances:

     An employee’s actions involving intentional and/or disregard of safety toward customers,


    employees, or the Company and its property. This is applicable when an employee has
    knowledge of and/or intentionally disregards procedures or policies. In cases where an
    employee has knowledge but still committed an error the report may be accepted as long as
    it is determined that the event was not intentional and the criteria listed herein are met.
     An employee committing a series of errors (meaning more than one) that demonstrates a
    general lack of care, judgment and professionalism. Management retains the discretion to
    review and interpret each situation and determine if that situation demonstrates a lack of
    professionalism, judgment or care. Managers should consider the risk associated with the
    event and the nature and scope of actions taken as a result of all previous events.
     An employee failing to report incidents promptly. For example, when an employee delays
    making a report in a reasonable time which shall be within 24 hours. However, reports should
    be submitted as soon as possible after the employee is aware of the safety error or close call.
     An employee failing to honestly participate in reporting all details in an investigation covered
    under this policy. For example, an employee fails to report all details or misrepresents details
    associated with an event or withholds critical information in his/her report.
     An employee’s actions involving criminal activity, substance abuse, controlled substances,
    alcohol, falsification or misrepresentation.

    1.2 Disciplinary Action Policy and commitment of Accountable Management


    Management is accountable for the safety performance of the company activities, products and
    services within the scope of their responsibilities. The above statements confirm my commitment
    to establish an appropriate disciplinary action management framework at (insert company name)
    and ensure that they promote a companywide safety culture.
