SOMEDAY.
1. When collecting client stories in the field, the staff will use the Client Success Story Form (Rev. 02-02-17). The form
includes basic information including the client’s name, location, business (sari-sari store, hair salon, etc.), Center
Name, loan amount, etc.
2. The form has questions which will be used to interview the clients and to learn about their businesses. We like to
learn as much as we can about the client’s personal life, their business and their loan, so be as thorough as you can
when interviewing clients and answering these questions. They should depict any struggle/s the client has faced
with their family, business and finances, and then reflect how a loan may have changed these aspects of their life.
3. When interviewing a client, they must also consent to having their story and photo used on the internet, so they
must sign the Universal Release Form (Photo/Video/Testimonial Release form).
1. When you are ready to email, make sure that you have encoded the answers in the MSWord file. This is to ensure
that the information is readable, clear and correct. This is also to avoid conflicts in the future. “Save As” the file
using the client’s name.
2. Transfer the photo from your digital camera/cellphone to the computer. Change the file name to the client’s
name. PLEASE DO NOT PRINT THE PHOTO. PLEASE DO NOT SCAN THE PHOTO and DO NOT INSERT THE PHOTO IN
THE MSWORD. We will not credit your submission if there is no photo included or if the photos are scanned,
attached in the MSWord.
3. Kindly scan the signed Universal Release Form since we need a copy of it, but you must keep a copy for your
records.
4. In your email cover letter, make sure your branch name and applicable month is indicated. Attach the (1) Client
Success Story Form, (2) scanned copy of signed Universal Release Form, and (3) client’s photo to your email. Send
