Dear Sir/Madam,
I am writing this letter to request to urgently admit Ms Olsen, a resident of Golden Pond
Retirement Village, who has had a sudden onset of pain in her shoulder, chest and neck for the
past three days.
Ms Olsen couldn't have her evening meal as she was feeling slightly nauseous. Due to pain in
her shoulder and neck, she was having trouble sleeping despite being given 1 Normison and 2
Panadol tablets. When she was rechecked 45 minutes later, she complained of persistent chest
pain and was found to be distressed, pale and sweaty. Additionally, her BP was 190/100.
Hence, she had to be shifted immediately.
Today morning, owing to Ms Olsen feeling generally weak and tired, she was confined to bed.
Her GP was called who said would visit tomorrow after surgery. However, since her condition
worsened all of a sudden, we could no longer wait.
Please note, Ms Olsen is a patient of hypothyroidism, hypertension and glaucoma and takes
daily medications for the same but is allergic to penicillin. She is also arthritic in her hands. The
details of all her medications are attached to this letter.
Being her night nurse, I am well aware of her health condition, so should you have any queries,
