Under the powers vested in rule 12 of the Tamil Nadu Government Servants Family
Security Fund Scheme sanction is hereby accorded for the payment of lumpsum benefit of Rs.
1,50,000/- (Rupees One Lakhs and Fifty thousand only) to Thiru/Tmt:___________________________
Wife/Son/Daughter/Father/Mother/Legal Heirs of Thiru/Tmt _________________________________
of this Office who expired on ___________________ while in service.
2. Thiru/Tmt_________________________________was admitted in the scheme, with

effect from _____________ and has been contributing Rs. 1 every month from the date Rs.3 every
month with effect from 1.4.1985 Rs. 5 every month from 1.7.1989 and Rs. 10 every month from
1.7.1991 and Rs.20 every month from 1.7.1996 and Rs. 30 every month from 01.02.2006.

“2235 Social Security and Welfare – 60 Other Social Security and
