PROJECT PROPOSAL

Project Title: Hypertensive and Diabetes Health Club in Purok Green Valley

Venue: Purok Green Valley, Brgy. Ubaldo Laya, Iligan City

Implementation Date and Time: October 26, 2018 and every Last Friday of the Month at
9:00 AM to 11:00 AM
Source of Funds: COMMED Funds of Team GV of Class 2020
Target number of participants: Around 70 percent of the Hypertensive and Diabetic
residents of Purok Green Valley
:

Rationale/Background:
Hypertension and diabetes have the highest frequency among the non-communicable
chronic morbidities in Purok Green Valley. There are a total of 26 cases of hypertension in the
Purok; out of this only 12 have controlled hypertension (through maintenance medications), 8
individuals have uncontrolled hypertension (incompliance to medications) and the remaining 6
individuals don’t take any hypertensive drugs. There are also 5 cases of diabetes in Purok Green
Valley, 4 of these have medications.
To address the problem, the MSU-COM Class 2020 Team Green Valley proposes the
formation of a Hypertensive and Diabetic Club in Purok Green Valley.

General Objective:
To reduce morbidity, mortality and disability due to chronic lifestyle related non-
communicable diseases specifically hypertension and diabetes mellitus through a group of
coherent interventions designed to prevent or manage these chronic conditions using a systematic
and multidisciplinary approach

Specific Objectives:
1.To identify persons at risk and those with hypertension and diabetes mellitus
2.To establish a Hypertension and Diabetes Club in collaboration with the Department of Health
and the Local Health Center
3.To aid Hypertension and Diabetes Club in the development and promotion of activities in line
with improvement of these health conditions
4.To establish a community Health Station provided with tools for BP monitoring and glucose
level measurement
5.To instill the importance of self-management and discipline by patients through lifestyle
modifications and medications
6.To achieve improvement of health by the end of 2018
Strategies:
1. Every families are given brochures containing information about common misconceptions on
Hypertension and Diabetes and the right management for people who are known hypertensive
and/or diabetic.
2. Community Hypertension and Diabetes club in collaboration with the DOH and the assistance
of local health centers in identifying individuals with hypertension and/or diabetes in the
community.
3. A certain Station in the community shall be established which is maintained by assigned
