3. Reading of the Minutes of the Previous Meeting
ASDS Daisy Z. Miranda read the minutes of the previous meeting. Some corrections
were made to some of the details in the minutes.
Ms Glecy Columna, EPS English, moved for the approval of the agenda. The body
seconded it.
The SDS inspired the group with a quotation, ”Be who you are” you are not
everyone’s cup of tea.

4.4 1. HUMAN RESOURCE AND DEVELOPMENT

2. SCHOOL MANAGEMENT MONITORING AND EVALUATION

As per Regional Memorandum 596 s. 2019 there will be


conduct of Field Visitation and Monitoring and Evaluation
(M&E) of Teaching and Learning Delivery (TLD) Programs and
Activities starting 3rd week of September- October 2019 on
selected schools. Visiting teams include staff from BLD-TLD and
Regional Office- CLMD. In line with this, all public secondary
and elementary teachers, school heads and supervisors need
to accomplish and complete the survey links below from
September 6-17, 2019.

-E-SIP Appraisal by the Division Appraisal Committee-Ongoing

SBM Validation of all schools will be conducted on October -


December 2019.

3. PLANNING AND RESEARCH

Submission of Research Proposals for BERF funding is still open.

4. SOCIAL MOBILIZATION AND NETWORKING

Before entering into partnership with private companies,


please make sure that they have an ATTESTATION on- hand.
This is to ensure that all partners have their documents
screened first by the Social Mobilization Unit.

NO FUND- RAISING ACTIVITIES shall be conducted in response


to the Regional Director’s directive that discourages all SDSs to
approve fundraising activities, particularly pageantry type of
activity

There will be a scheduled monitoring of the SMN Unit that aims


to check and validate the partnership documents of all
schools and the status of their partnership with their private
partners.

Please be reminded of the submission of DPDS for 3rd Quarter


through uploading of the xls. file (Excel) through the link

There will be a Division PTA Forum this coming September 23 –


24, 2019. Venue to be announced on a later date.
GUIDELINES ON THE PREPARATION OF DOCUMENTS FOR PTA
INITIATED ACTIVITIES

To effectively implement DepEd Order No. 54, s. 2009 entitled


“Revised Guidelines Governing Parents- Teachers Associations
(PTAs) at the School Level”, specifically policy governing fund-
raising activity initiated by PTA, this Office releases the
attached enclosures

Checklist of Requirements for PTA Initiated Activity (School-


Based) and PTA Initiated Off- Campus Activity

Submission of Activity Proposal (School Based) shall be 15 days


before the scheduled conduct and for Off- Campus Activity
shall be one (1) month before the scheduled conduct (DO No.
66, s. 2017)

All PTA documents shall not bear the logo of any DepEd
Offices.

All documents to be submitted must be formatted as follows:

Paper Size - A4

Font Style - Century Gothic

Font Size - 11

Margin - 1” on all sides

5. EDUCATIONAL FACILITIES
