This document contains a worksheet for assessing occupational health risks across multiple job tasks. The worksheet was designed by Occupational Health Clinics for Ontario Workers to be used with their Quick Exposure Check for evaluating various exposures, including back/shoulder stress, wrist/hand stress, neck stress, vibration, work pace, and stress. Users are instructed to only complete assessments if they have received appropriate training. The blank worksheet allows for evaluating up to 3 job tasks with each task having photos and ratings for different types of exposures.
