AFFIDAVIT OF LEGITIMATION
WE, DANIEL O. FLORES and ROSEMARIE D. FLORES, both of legal ages, husband and wife, respectively, Filipino citizens, and residents of Brgy. I, San Fernando, La Union, after having been sworn to an oath in accordance with law, do hereby depose and say:
1. That we are the parents of BIMBY D. FLORES, who was born on February 14, 2019 in Lorma Medical Center, San Fernando City, La Union;
2. That our son was born out of wedlock;
3. That when our son was conceived, we both possessed no legal impediment to marry each other;
4. That we contracted our marriage on May 15, 2019 before Hon. Hermenegildo A. Gualberto, Mayor of City of San Fernando, La Union;
5. That by virtue of our marriage, the aforesaid child is legitimated by operation of law.
6. That we are executing this Affidavit to attest to the foregoing facts, for purposes of complying with the requirements in applying for the legitimation by subsequent marriage of our son, BIMBY D. FLORES and for all legal intents and purposes.
