is hereby approved. Please be informed, however, that you are hereby confined to the following terms and
conditions:

1. That you agree to serve the company for a period of (__) months
and (__) days commencing from the first working day after the
seminar/training/conference/travel abroad;
2. That the return service period is cumulative and shall be served in the chronological sequence
based on the amount incurred by the company for such training, seminar or conference, or the
travel expenses / fare shouldered by the company for your training.
3. That should you consider resigning from the company or should your services be terminated for
cause before the said period expire, it is understood that you shall refund the company in the
amount equivalent to twice the amount spent for the training, seminar or conference, or travel
expenses plus liquidated damages;
4. That you are required to submit a written report on the coverage of the
seminar/training/conference/travel abroad expenses / fare within ten (10) days after the
termination of the said undertaking;
5. That you are required to turn over to the company the accomplishment report after the
training/conference/travel abroad and the like.
