AFFIDAVIT OF UNDERTAKING

___________________________________, Philippines, after having been duly sworn to in accordance
with law, depose and say:

That I applied for a job with ______________________________________________________,


with business address at ___________________________________________, binondo manila,

That I understand that I am being employed as _________________________ on the condition


that I shall work with the named firm for a minimum period of 5 months on a probationary status;

That I also understand that my probationary employment is an exception to the general rule
limiting the probationary period to 5 months, since I have agreed to a longer probationary period than 5
months considering that the position I will be assigned to requires extensive and intensive in-house
training which shall include classroom situation and on-the-job practicum;

That I fully accept this condition and am well aware that the training I will undergo requires
specializations, skills, experience and certain qualification which cannot be fully grasped nor the skills
satisfactory learned within the usual period of 5 months;

That should I fail to meet the minimum standards and expectations set within the probationary
period and/or after my training, the same shall be a ground for my termination;

That should I resign within the probationary period or be terminated therefrom, I undertake not to
begin/join any business endeavor or be employed by any entity or individual requiring skills and training
that I have undergone for 5 months after my resignation or termination; otherwise, I shall compensate my
herein employer triple the cost of my training or last salary;

That should it become necessary for my herein employer to retain a lawyer to enforce the herein
undertaking, then I shall pay the attorney’s fees, costs of suit and the costs of my training;

That the conditions of my probationary employment have been explained to me and I understood
and agree to accept them with full knowledge of my legal rights.

That I am executing this affidavit freely to attest to the above facts for whatever legal ends the
same may serve.
