Dear Dr. Cuyugan,

We request your good office to excuse us from wearing the school uniform for the following
reasons:

For this 2nd semester, we are taking the IAC Review Subjects. The review classes are held in PRTC
Review Center, Recto St., Manila. The primary purpose of going to Jose Rizal University during
school days is for taking the quizzes and major examinations of the IAC subjects. We would also
like to study in the library during our free time.

In the light of their obligations as students, we pledge to wear the School I.D at all times and
subject us to a suitable dress code.

We are hoping for your kind consideration. Attached herewith is the list of students applying for
uniform exemption.
