We at Nu Skin Enterprises Philippines, LLC (Nu Skin) would like to ensure the safety of your commission and its timely
payment. Simply submit details of your bank Account Number (available banks listed below) and your commissions will be
directly credited to your account based on Velocity’s commission payout schedule. Please fill up the form with the appropriate
information, completely and accurately. Distributor certifies that the information given is true and correct. Nu Skin cannot be
held accountable for incorrect crediting of commissions due to misrepresentation on the bank information provided by
the Distributor.
