3.) In view of the initial screening conducted by this Office last February 23, 2016, the list of
teachers who will proceed to the evaluation of pertinent documents can be found in the
Enclosure No. 1.
4.) HT III-Science and HT III-English/HT I-Filipino applicants are enjoined to submit the
following documents relative for the filling up of the aforementioned items to Mrs. Roshenar
V. Cruz, Administrative Officer IV, not later than May 17, 2016, 12:00 noon and May 18, 2016,
12:00 noon respectively.
a) Letter of intent
b) Transcript of Records for baccalaureate degree and post-graduate degrees
c) Ratings obtained in the Licensure Examination for teachers (LET/PBET) and PRC
professional identification card
d) Performance Rating (Last 3 Ratings: SY 2015-2016; SY 2014-2015; SY 2013-2014)
e) Twenty four (24) hours of relevant trainings certificates
f) Updated Service Record
g) Certificates of Seminars attended
h) Last CSC Form 33 (Appointment)
i) Outstanding Accomplishments (Certificates; a-e, DepEd Order No. 42, Series 2007)
j) Omnibus certification of authenticity and veracity of all documents submitted, signed by
the applicant (copy attached).
5.) All interested applicants for Guidance Counselor III, Administrative Assistant III and
Administrative Aide III are enjoined to submit the following documents relative for the filling
up of the aforementioned items to Mrs. Roshenar V. Cruz, Administrative Officer IV, not later
than May 19, 2016, 12:00 noon.

c) Ratings obtained in the CSC Professional Eligibility (Guidance Counselor III) CSC Sub-
Professional Eligibility

k) The documents listed must be properly labeled inside a long brown envelope with applicant’s
name, present position/designation, school and the position being applied for, written on the
extreme left corner of the envelope.
