Is the person authorized/trained and competent to work at height?
Is the scope of work properly communicated to the permit receiver?

Is the Risk Assessment conducted at work location?


Is the area cordoned off/barricaded?
In case work is carried out in area which requires energy isolation- is energy isolation done?
Is the person received training on RF-EMF Safety?

Is the person not wearing an implanted medical device (pacemakers and defibrillators)?

Is the Supervisor aware of sector shutdown / power down procedures if temporary access is required in front lobe of
antenna?

In case of overexposure, is the person aware of the Incident reporting mechanism?


Are proper scaffold/Ladder/etc, used for height work? Is it secured against movement at top &base?
Is fully body safety harness with double lanyard provided to height workers?
Are safe anchoring points available for securing the safety harness?
Is safety helmet with chin strap provided?
Is the height worker physically fit to work?
