    AFFIDAVIT OF LOSS

    I, MAE FLOR R. BALUIS, Filipino, of legal age, single and a resident of


    Purok 6-A, Barangay Magdum, Tagum City, Davao del Norte, Philippines,
    after having been duly sworn to in accordance with the law, do hereby
    depose and say THAT;

    1. I was issued a PASSPORT in my name by the Department of Foreign


    Affairs, Region XI, Davao City;

    2. I was an Overseas Filipino Worker at Al Ain, United Arab Emirates from


    2011-2017;

    3. The said passport was safely kept in my house at Al Ain, United Arab
    Emirates;

    4. Sometime on 2016, the said passport was lost after I transferred to


    another house of the same street;

    5. I exerted diligent efforts to locate it but the same proved futile. For this
    reason, it is considered lost beyond recovery;

    6. I am executing this Affidavit to attest to the truth and veracity of the


    foregoing statements and to apprise the proper authorities concerned
    on the matter for their information and guidance for the issuance of a
    new passport and whatever legal purpose/s this may serve best.

    lost after the typhoon Yolanda struck our home town;
