NC STEM Academy does not provide liability insurance for its summer camp participants. By participating in the summer camp, attendees release NC STEM Academy from all liability for any injuries sustained. This release applies to both known and unknown potential future injuries. NC STEM Academy will notify parents of any incidents or accidents and fill out an incident report. Parents must inform NC STEM Academy of any student allergies or special needs before the start of camp. NC STEM Academy will not release a child without confirmation of the picking-up adult and has parents' contact information on file.

Liability Disclaimer

SUMMER CAMP
NC STEM Academy does not provide liability insurance for the protection of children, parents,
visitors, or others who may participate in the summer camp. In consideration for your
participation in said Summer Camp, the attendee does hereby release and forever discharge NC
STEM Academy and its teachers, board, and employees, jointly and severally from all actions,
causes of actions, claims and demands for, upon or by reason of any damage, loss or injury, which
hereafter may be sustained by participating in the Summer Camp.
This release extends and applies to, and covers and includes, all unknown, unforeseen,
unanticipated and unsuspected injuries, damages, loss and liability and the consequences
thereof, as well as those now disclosed and known to exist.
NC STEM Academy is responsible to notify parents of any incidents or accidents that may happen
while your child is in our care. An ‘incident report” will be filled out to inform parents of how it
occurred. Should any child have allergies or need specific special care or regular medication, the
management MUST be made aware BEFORE the start of the summer camp! We do not accept
children without having your full contact details and a second contact in case of an emergency.
We do not let a child leave the premises unless we have been given confirmation on who is
picking the child up.
I hereby agree to indemnify NC STEM Academy and its teachers, board and employees, joint and
severally from all actions, causes of actions, claims and demands for, upon or by reason of any
damage, loss or injury, which hereafter may be sustained by participating in the Summer Camp.
It is further understood and agreed that said participation in the Summer Camp is not to be
construed as an admission of any liability and acceptance of assumption of responsibility by NC
STEM Academy, jointly and severally, for all damages and expenses for which NC STEM Academy
becomes liable because of any alleged act of the parade participant.
