To find your Form 1095-A online, follow these steps: 1. Log into your Marketplace account and select your 2017 application from the menu. 2. Select "Tax forms" from the left menu and click "Download [PDF]" to save the form to your computer. 3. Open the saved PDF file and print a copy for your records. You may have multiple 1095-A forms if you changed plans during the year, but use the most recent one with the same policy number.

Form 1095-A online

 Learn more about health coverage and your federal income taxes.

 Find out how to use your Form 1095-A once you have it.
STEP 1 Log in to your Marketplace
account.

Click your name in the top right


STEP 2
and select My applications &
coverage.

Under “Your existing


STEP 3 applications,” select your 2017
application — not your 2018
application.

STEP 4 Select Tax forms from the
menu on the left.

Under “Your forms 1095-A


STEP 5
for tax filing,” click Download
[PDF] and follow these steps
based on your browser:
• Chrome users: The
downloaded PDF will ap-
pear at the bottom of the
screen. Click to open.
• Internet Explorer users:
Click Save at the bottom
and then Open.
• Safari users: When the
pop-up appears, select
Open with and OK.

Save the file to your
computer.
• Chrome users: Click the
download icon at the top
STEP 6 right (it looks like a
downward facing arrow).
Locate where you want to
save it on your computer,
and click Save.
• Internet Explorer users:
Click Save at the bottom
and then Open.
• Safari users: Click File in
the top left, then Save as.
Locate where you want to
save it on your computer,
and click Save.

Open your saved file. Click File in


the top left, and Print to print a

Have more than one 1095-A?


You may have more than one
if your household enrolled in
more than one Marketplace
health plan or if you reported a
life change during the year. But if
the Marketplace-assigned policy
number in Box 2 is the same
among the forms, use the most
recent one.
