SCHEDULE 3
SCOPE OF SERVICES
1. GENERAL
The Scope of Services during the term of the contract, the Subcontractor shall provide,
implement and follow the government regulations:
1. Guide and refers to Law No. 32 Year 2009 on Environmental Protection and Management
2. Guide and refers to PP101 Year 2014 on Waste Management of Hazardous and Toxic
Materials.
3. Guide and refers to ministerial regulation number 21 Year 2017 on Drilling Waste
Management of Mud Waste and Cutting Waste on the Geothermal Activity
4. Guide and refers to UKL UPL of Sorik Marapi Geothermal Power and Environmental Permit
The general objectives/purposes of the waste management services are:
To provide handling equipment and manpower drilling cuttings and muds.
To provide transportation services for drilling cuttings.
To provide transportation services for drilling mud waste.
To provide transportation services for domestic waste.
To provide service to contain hazardous / B3 waste.
To provide general services on the drainage system to keep it clean.
To provide optional services related to waste management.
2. TECHNICAL SERVICES
The Subcontractor shall provide Waste Management Services in support of the GDAP. The Work
shall include, but not be limited to provision of all personnel, equipment and supplies in support
of the Work. Subcontractor will also furnish all technical and professional services as needed
for quality control and any of all other items necessary to perform the Work unless specifically
stated in the Agreement.
Subcontractor shall have applicable certifications from Government of Indonesia agencies (GOI)
such as Surat Keterangan Terdaftar (SKT) from Direktorat Jendral Minyak dan Gas Bumi and
Direktorat Jendral Energi Baru, Terbarukan dan Konservasi Energi (EBTKE) and also applicable
permit related to waste management required for the services provided by Subcontractor to
GDAP.

Throughout the life of the Contract, Subcontractor is expected to liaise and cooperate with all
other Subcontractors supplying services, materials, and personnel to GDAP. Teamwork is
essential for safe and efficient operations and Subcontractor is expected to be fully team
oriented at all times.

Subcontractor shall provide Standard Operation Procedure (SOP) for these processes:
pre-treatment process and on-site treatment,
packaging,
Removal and transportation as well as final management of Hazardous and Non-
Hazardous waste (especially powder and drill mud as industrial raw material (cement
etc.), raw materials Construction and or in land field).
4. PERSONNEL
Subcontractor will provide qualified and experienced personnel sufficient in number and with
appropriate qualifications to maintain and operate the equipment on site, and process and
interpret data as required. Subcontractor 's personnel shall work in conjunction with the
GDAP’s Drilling Subcontractor and under the direction of GDAP’s Wellsite Representative. All
field personnel must be fluent in English and be able to communicate the specifications,
operating procedures, parameters and capabilities of the services and equipment for which
they are responsible. GDAP reserves the right to interview proposed personnel to assess
experience and fluency in English.
Only personnel nominated in Section 4.8 will be utilized on GDAP’s operations unless prior
written approval has been obtained from GDAP’s Representative. Subcontractor shall submit
resumes (CV) for all nominated personnel.

For 24-hour operations, it is expected to have 2x12 hours shift of one (1) Supervisor–wellpad,
one (1) telehandler driver, one (1) vacuum truck driver, one (1) light vehicle driver and two (2)
local helpers. All personnel will be furnished with complete personal protective equipment
(PPE) required in accordance with GDAP’s PPE policy and all hand tools necessary to complete
their tasks.
GDAP will provide limited mess and accommodation for Subcontractor’s designated field
personnel. GDAP will not be responsible for messing and accommodation for Subcontractor ’s
assigned non-field personnel.
Subcontractor shall provide the required personnel on an as required basis, and shall be
responsible for their mobilization and demobilization to and from the Client’s Sorik Marapi
Geothermal Power’s drilling site as required. The Delivery Point (TBA).
Subcontractor ’s personnel shall attend all safety meetings (compulsory) and pre-job meetings
as appropriate.
4.1. Supervisor
Minimum qualifications and experience required for Supervisor shall be:
Minimum of eight (8) years practical waste management experience in oil and gas
and/or geothermal industries.
Minimum of four (4) years as a supervisor in oil and gas and / or geothermal waste
management service experience (preferred personnel with geothermal waste
management service experience).
Supervisor shall be responsible for:
establishment and general supervision and management of the Subcontractor ’s
Waste Management Services at the GDAP’s Project Site,
all operational aspects of Waste Management operations on the rig or basecamp,
personnel and equipment safety and loss control, and
Onsite inventory management, etc.

4.6. Septic Tank Truck Driver for domestic waste (black water / greywater)
Septic Tank Truck Driver for domestic waste (black water / grey water) shall be
responsible for actual transportation for drilling mud waste and any optional services
that relate to waste management.
Minimum qualifications and experience required for Septic Tank Truck driver shall be:
Minimum of three (3) years with the respective type of equipment.
Fully certified in compliance to relevant GOI regulations

Technical Support Supervisor is the primary point of contact between GDAP and
Subcontractor. Technical Support Supervisor shall be proactive in the pre-job planning
process. He/she shall also make recommendations, to GDAP’s Drilling Team, intended to
improve efficiency and eliminate risk.

5. EQUIPMENT
Subcontractor shall provide the required equipment on an as required basis, and shall be
responsible for mobilization and demobilization of this equipment to and from the Sorik Marapi
Geothermal Power drilling site as required. The Delivery Point of each contract (for
Mob/Demob) to Jakarta with optional to Ende site location.
Subcontractor is required to have a Preventative Maintenance Plan for equipment that meets
original manufacturer’s recommendations and shall ensure that such preventative
maintenance is undertaken in accordance with that Plan. In addition, Subcontractor shall
provide sufficient spare parts at the site to maintain and repair all equipment for the duration
of the Drilling Project.
Subcontractor shall have implemented a Quality Assurance Program (QAP) and shall supply
Services and Materials in accordance with said QAP. Such QAP shall be either the API Quality
Assurance Program Q-1; International Organization for Standardization Guidelines ISO 9000-
9004, ISO 9001; or equivalent. Subcontractor shall provide a copy of Subcontractor ’s quality
control program and demonstrate its implementation upon GDAP’s request.
Subcontractor warrants that all equipment used has been inspected prior to mobilization,
and during the Contract term, as required.
Subcontractor shall liaise and cooperate as required with GDAP’s other Subcontractor s when
preparing to commence the Services. Subcontractor shall cooperate to ensure installation
and commissioning of Subcontractor ’s equipment proceeds in a safe and effective manner,
with no interruption to GDAP’s operations.
Equipment to be provided:
5.1. Manitou Telehandler (2 units)
Subcontractor shall provide 5.1. Manitou Telehandler with the following minimum
specifications:
1700 Lbs capacity multi-purpose 0.34 Cubic Meter bucket.
Tires or track drive is acceptable.
Full ROP cabin.
Bucket height must be matched to Item 4.2 Dump Truck to allow side loading of the
dump truck on level ground.

6. CONSUMABLES
Subcontractor shall be solely responsible for managing its supply of materials to ensure
availability as needed by GDAP and to ensure that quality of the material is maintained.
Subcontractor shall ensure that at all times the material stock never depletes through loss,
damage or otherwise to such an extent that an adequate inventory of materials is not available.
Material containers including drums and other similar containers required by Subcontractor
will remain the property of Subcontractor. GDAP will not be responsible for the disposal of any
Subcontractor provided drums or similar containers.
Subcontractor shall supply materials necessary to support waste management services work
and all other consumable items required for operations.

7. DELIVERABLES
A daily report (soft copy) shall be delivered whenever waste management service
operations take place. The report will be required on a midnight-to-midnight basis, and
will include the following data, but not limited to:
a) Location identification giving times and dates of services, well name, field, and rig
name
b) Names and Job titles of all Subcontractor ’s Personnel providing the service, and dates
and time of departure and return from the worksite.
c) Operations summary, equipment operating hours, and maintenance activities.
d) Total amount per day of drill cuttings and fluids waste transported and domestic
waste.
e) Fuel, oils, and materials supplied, consumed and remaining balance
f) Report of any equipment failure of Subcontractor ’s equipment and/or Work and
reason(s) there of A job detail with time breakdown
g) Daily cost with itemized cost breakdown
An End of Well Report shall be furnished to GDAP's Jakarta office in conjunction with the
invoice for each well. Subcontractor shall provide the report in both hardcopy and digital
format. Reports are expected to be 100% accurate. The report content and format to be
used shall be approved by GDAP prior to use. Final invoices will not be paid until End of
Well Report has been correctly and properly submitted. End of Well Reports must be
submitted within fourteen (14) calendar days after the completion of each well and will
include the information listed below:
a) Job plans and job execution summaries
b) Health, Safety, and Environment including HSE stats, near miss, and incident reports
c) Material and equipment costs (daily and total for the well)
d) Consumables usage summary. This shall show quantities at the beginning of the well,

goods delivered, goods used, goods transferred, goods returned, and the end of well
balance. The quantities of goods delivered, used, transferred, or returned shall be
identified by date that the balance was adjusted.

8. WORKCYCLE
The Subcontractor shall provide Personnel when requested by GDAP to work a (12) twelve-hour
rotation and additional work may be required from time to time within occupational health and
safety requirements and such additional work shall not incur additional costs. Personnel shall
not work more than a three (3) week on three (3) week off rotation. GDAP shall in form the
Subcontractor if it requires additional Personnel to perform.
