Job Summary:

Primary Tasks:
1. Ensures that case management and caseload inventories at the provincial level are maintained and
updated.
2. Assists in the development of provincial/local inter-agency mechanisms for the provision of
interventions on cases that are beyond the resolution capacities of the City/Municipal Links and LGU
social workers.
3. Works closely with the C/ML in conducting case management processes for clients at risk.
4. Assists in organizing and maintaining pool of experts at the provincial level to provide legal,
psychiatric, psychological and medical interventions, if necessary.
5. Ensures the availability of directory of services that may be appropriately tapped for case referrals.
6. Assists the focal person in convening case conferences in POOs using inter-disciplinary and multi-
disciplinary approach.
7. Monitors the e-case management system or any other equivalent case inventory system.
8. Prepares reportorial requirements for the Regional and Central Office and if appropriate, for other
requesting parties.
9. Performs other related functions as may be assigned by the SWO IV.
