SUBJECT: Report of Broken Fasteners on TDS-11SA Motor Housing Guard

PURPOSE: To Advise Operators to Inspect Motor Housing Guard

EFFECTIVITY: All TDS-11SA Top Drive Drilling Systems

During initial installation of a TDS-11SA Top Drive Drilling System the rig crew noticed that six
of the ten fasteners, which attach the guard to the motor housing, were broken. Further
inspection indicated that the system bail had made contact with the guard mounting feet
causing side loading and shearing of the fasteners. No injuries were reported as a result of
these broken fasteners.

The Motor Housing Guard (P/N 120887) attaches to the TDS-11SA Motor Housing at four
locations using ten 1-8 x 2-1/2” Grade 5 fasteners. Two of the mounting feet attach to the TDS
Carriage Assembly, while the other two mounting feet attach to the Transmission/Motor
Housing and Swivel Assembly, adjacent to the system bail pivot point, as shown in Figure 1.

In reviewing the design of the TDS-11SA Varco has determined that during rig up, when the
hook is attached to the bail and the top drive is lifted into vertical position, it is possible for the
bail to pivot far enoug h to make contact with these two mounting feet.

Varco has developed a modification to the guard mounting feet to eliminate any possibility of
fastener damage. The Bail/Wedge Retrofit Kit (P/N 30171317) provides newly designed
mounting supports for the upper guard, associated fasteners and kit instructions for
installation. This kit is being provided free of charge to all affected rigs and can be ordered
with a no cost purchase order.
