ACTIVITY COMPLETION REPORT IN MATHEMATICS


SY : 2018 - 2019

Mathematics is a challenging subject. It requires great patience and


sacrifice to obtain its objectives. Traits that each mathematics teacher should possess
in order to become successful in teaching math to the students. To become successful
in this endeavor, there should be program of activities and action plan that will serve
as a guide on the appropriate actions to obtain the objectives.
School year 2018 – 2019 was a more fruitful one for the mathematics
department. We have organized our mathematics club and elected set of officers for
the year. We have participated on the different contest in Mathematics like Division
Math quiz and the METRO BANK – MTAP – DEPED MATH CHALLENGE and have two
grade levels that qualified to Division MMC challenge namely grade 8 and grade 9 and
fortunately landed top 7 for grade 8 and Top 10 for grade 9. We also have conducted
mathematics culmination program together with the science department where
different mathematics contest where initiated. The average MPS for mathematics have
also increased significantly from the first quarter to the fourth quarter. We have
conducted pre-test /post-test for numeracy and earned reasonable increased on
students performance on the four basic mathematical operations. All the mathematics
teachers have participated the INSET activities initiated by our school head like RPMS
PPST roll out, Item Analysis, and Research Training. We also have secured the test
item bank for mathematics from the region but unfortunately, only the third and
fourth quarter were available.
The Mathematics department was also very supportive to the portfolio
celebration on that year as we have constructed, together with the science department,
portfolio booth in every quarter showing outputs of students as products of the
lessons that were given to them.
Indeed, the school year 2018 -2019 had given us success in our
endeavors, but we believed that our accomplishments are not enough, there are still
more areas to be improved and developed and these give greater challenge to exert
more effort to the attainment of our goals, to make mathematics more appealing, more
productive and more helpful to the students and to the school. A challenge that each
mathematics live by their mind and heart.

Hindang National High School Alumni Batch 1998 represented by their


Batchmate Ms. Lolita Acogido has donated cash in the amount of 2,000.00 to
purchase new curtains for the SSG office. Lolita has a soft heart for the SSG
office because she was once a student body organization senator.

III. ACCOMPLISHMENT OF OBJECTIVES


To purchase new curtain for the SSG office.

IV. STRATEGIES
The Brigada Eskwela Coordinator sent a solicitation letter to the different batches of
alumni, since the school has recently celebrated the third grand alumni last MAY 9, 2018. Thanks
to the positive response of the Batch 19948 they were able to gather the said amount and was
sent to their batch mate who is residing within the poblacion..

Our school is very lucky and fortunate because one of our


teacher was married to an American Citizen who is now residing in
Chicago, USA and has come back home as Balikbayan and has
donated Php 1,500.00 for the snacks of our volunteers in the
Brigada Eskwela. The said amount was used to purchase 200 pcs
of bread at Php 5.00 each and softdrinks.

III. ACCOMPLISHMENT

 To provide snacks for the Brigada Eskwela during the first day of activity.

IV. STRATEGY: Sending a solicitation Letter to Mr. Labis himself


through his wife Ms. Janna C. Albesa
