Camper Forms Completion Guide

The following forms and documents are required for all campers. All forms can be accessed through your own
password-protected CampInTouch account. Some forms must be submitted online while others must be returned via
email or fax. Please read carefully and note the different deadlines. Please call the Camp Office with any questions
at 781-320-1320.

Form How to Complete Due Date

Health History Form Online Due: within 10 days of registration

This form must be completed online and is required within 10 days of acceptance for all campers. Please have all insurance
information, physician and dentist names accessible as you complete this form. If your child is a returning camper and you
previously completed this form, you will only need to update as necessary.

Physician’s/Immunization Form Email, mail or fax back Due: within 10 days of registration

Required: NDC requires that each camper must have an updated annual physical exam within 12 months of camp attendance.
Download our Cover Sheet to upload the physical or email ([email protected]), mail or fax (877-325-8736).

Medication Administration Form Online Due: June 1

This form is required for all campers who will bring prescription medication to camp and must be signed by their parent and
physician.

Additional Options Online Due: June 1


Bus Transportation: offered AM or PM only or round trip from Brookline, Needham, Newton & Westwood
AM Care: 7:30 am-8:30 am: You may choose individual days or the entire week.
PM Care: 4:00 am-6:00 am: You may choose individual days or entire week.
Stay and Play: 2:00 pm-4:00 pm: Offered to our 9:00 – 2:00 campers.

Authorized Grown Ups Form Online Due: June 1,

This form is required to authorize individuals other than parents to pick up campers. Please make sure to complete this if at
any time a grandparent, friend, babysitter, or carpool driver will be picking up your child.

Lunch Form Online Week before attending

Order lunch on a weekly basis, Monday – Thursday. Friday is an all camp cookout which is included in your tuition. For
ordering purposes, the deadline for menu selection is one week ahead. Menu and order form available June 1st.

Camper Profile Form Online Due: June 1


This form provides us with helpful information about your camper. The contents are shared specifically with your camper’s
counselors so they can get to know your child quickly and connect with them.

Camper Photo Online Optional

If you did not upload a photo of your child at the time of registration, please do so by June 1. A photo will help our
counselors and healthcare staff recognize and get to know your camper before he/she begins camp.
