    I, LAURENCE, of legal age, Filipino, married to Emelita Delim and a resident of


    Villa San Juan, Panicsican, San Juan, La Union, after having been duly sworn to in
    accordance with law hereby depose and say:

    That I am the registered owner of a motor vehicle which is particularly described


    as follows:

    Make & Type : Isuzu


    Motor Number : AJJ1KN7412
    Chassis Number : MPAUCR85HCT101075
    Plate Number : WTO 594
    MV File No. : 1380-00000767217
    Year Model/Color : 2013, SILKY PEARL WHITE

    As proof of ownership of the above-described vehicle I was issued by the Land


    Transportation Office, Official Receipt (OR) and Certificate of Registration (CR);

    That on August 22, 2018, I was driving said vehicle on a flooded area as I passed
    said situation, I noticed that my front plate was missing and nowhere to be found
    despite diligent search;

    Said plate were not confiscated by any government authorities for violation of R.A.
    4136 and other related laws;

    I am executing this affidavit to attest to the veracity of my statements and to


    request for a replacement of plate or authorization to use temporary plate to the
    Land Transportation Office (LTO), San Fernando City, La Union, Region i.

    IN WITNESS WHEREOF, I have hereunto set my hand this 30th day of August
    2018 at San Fernando City, La Union, Philippines.
