This document certifies that an employee completed authorized travel. It includes fields for the employee and director to provide their names and signatures. The employee must check one of the boxes to indicate if they strictly followed the approved travel itinerary, cut the trip short and refunded excess payment, extended the trip and submitted an additional itinerary, or deviated from the plan otherwise. Space is provided to explain any differences from the original itinerary and to provide evidence of completing the travel. The director then approves that the travel was undertaken based on the information and evidence provided.
