Collecting the data


1. Click on the recording template - sheet 1 tab at the bottom of the screen. Print the worksheet.
2. Click on the recording template - sheet 2 tab at the bottom of the screen. Print the worksheet.
3. Make photocopies of these two pages as required. Staple sets of the two pages together.
4. Each set of these two pages is used for the assessment of a student's overall progress.
Note: Data for the highlighted VELS Domains is required (data for non-highlighted
Domains is optional).

5. Collate the completed assessments.
6. Click on the Data entry tab.
7. For each student, enter the student's name, teacher's name, section/class information and
assessment results.
8. Students' assessment results entered in the Data entry worksheet are automatically aggregated
in the Summary Data for Webform worksheet.

Transmit data electronically to Central Office (if there are 10 or more PSD students)
9. Click on the Summary Data for Webform tab at the bottom of the screen, and follow the
instructions on how to transmit data to Central Office.

School Curriculum Survey (Special Settings) for PSD students
On that webpage, click on the link for PSD Webforms (principal's edumail login and password required).

- select the required calendar year.
