Assessable Spouse Election Form

This form allows a married couple to elect for one spouse to be designated the "assessable spouse" for tax purposes. The couple provides their names, addresses, estimated incomes, and date of marriage. They choose whether to allocate tax credits and the standard tax rate band as they have been, equally between the spouses, or entirely to the assessable spouse. Both spouses must sign and date the form.
