PLANNED ASSESSMENT

This template will be used throughout the topic of ‘I am me’. At the beginning of the topic the educator will print this template out
and attach it to a clipboard. This will allow easy access and the flexibility to write authentic data. By highlighting the focus child/
group each week the educator will be able to identify the children they need to focus on.
