ECC Checklist

Geo-tagged photographs of the proposed project site (taken last 30 days)

Maps of impact/ affected areas showing at least 1 km from the project


boundaries ( vicinity map)

Certification from Local Government Unit (LGU) proving the compatibility


proposed project with existing land use plan

Site development plan/ Vicinity map signed by registered professionals

Project/ Plan Lay-out signed by registered professionals

Schematic diagram of air pollution control facility (if applicable)

Organizational Chart in charge of environmental concerns

Proof of authority over the project site (e.g. land title, lease of contract, deed of
absolute sale, etc.)

Affidavit of No Complaint executed by the proponent

Waste Management Plan (hospital)

SEC Registration

Board Resolution Authorizing the person to sign and process ECC Application

Constitution and By Laws

Articles of Incorporation
