To ensure the safety of our students, The International School of Penang (Uplands) requires host families to be
aware that we are discharging students into your care. By completing and signing the form below, you are
agreeing to accept full personal responsibility for the boarder whilst in your care. Please ensure you fetch the
boarder from the Boarding House and return them to the Boarding House at the pre-arranged times. Boarders
should be back before 6:00 p.m. Sunday evening. If for any reason you are unable to look after the boarder,
please return the boarder to the Boarding House into the care of a Boarding House Parent.

We would be grateful if you could complete and return the form below to ensure that permission can be given for a
visit to go ahead. Thank you.
