Broken Window Broken/dilapidated ceiling


Slippery Pathway Open/incomplete perimeter fence
Blocked corridor Presence of stray animals inside the school campus
Heavy objects mounted on top of cabinets/ Presence of electrical posts/transformer near or
shelves within the school perimeter
Flooded area No system of release to parents during emergencies
Busted plugs/light bulbs/electrical facilities No posted emergency hotlines around the school
Exposed electrical wires Garbage area (Segregation of Biodegradable and
Protruding nails in chairs and tables non-biodegradable)
Broken door knobs Detached or peeled off GI sheet
Warning sign:Slippery pathways/corridors Broken toilet bowl and/or sinks
Plants mounted on the building railings Broken chairs/desks/tables
Flooding Blocked/no emergency exits
Exposed chemicals and liquids Unmounted cabinets/shelves
Lack/absence of storage for equipment Medical kits in every classroom

Dripping ceiling Condemnable building (i.e. very old structure,
