1. By the end of the semester, I would like to be able to complete at least 3 Canadian Neurological
Scale (CNS) on my patients with full confidence. I can measure by using a tally for every time I
felt fully confident and did not ask for assistance.
2. By the end of the semester, I would like to assist the other nurses and my peers more often and
keep my shift more productive. I can measure this by keeping track of the number of minutes in
which I find myself with nothing to do. At the end of the semester I would like less than 10
minutes for the whole shift of downtime (not including break or post-conference).
3. By the end of the semester, I would like to insert at least 1 catheter (in and out or indwelling) and
perform at least 1 more dressing change on a patient.

How will I achieve my goals? (activities, open-lab, text-based readings, journal articles, approved videos
etc.)

1. I can achieve this goal by reading more about the CNS, why it’s used and how to successfully
perform the assessment. I can do this by watching my peers as well as the other nurses perform the
assessment as well as watching videos. I can also ask the other nurses if I could perform a CNS on
their patient’s for extra practice.
2. I can achieve this goal by making an effort to ask the nurses if they need assistance. I can also ask
my peers or answer the call bells. Another way I can keep productive is to walk around to all the
client’s and ask them if there is anything, I can get for them or if they need assistance toileting.
3. I can complete this goal by first being prepared before the opportunities arise. I can do this by
reading the literature and watching videos on the process and making a list of the supplies I may
need. Then, I would let my instructor know that I am interested in practicing these skills and then
wait for the opportunity to arise. I can also let the nurses know that if they need a dressing change
or a catherization performed, to let me know.

Evaluation-prove how you have met your goals:

1. I feel as if I am more confident when performing my full assessments. Throughout the


semester, except for the day that the flashlight was not in the vitals cart, I have really only gone
in and out of my patient’s room once when completing my full assessment. On one occasion, I
had to go out of my patient’s room to get them water to take their pills. I feel as if I can
continue to develop this skill throughout the rest of the semester. I will continue to measure by
keeping a tally.
2. At the beginning of the semester, I would have a few notes or changes to my progress notes
from my clinical instructor. With the exception of a difficult progress note about a wound
change, there has been mostly one sometimes no changes to my note. I think this is considered
progress to my goal and hope that by the end of the placement there will be no more notes. I
will continue to count the amount of revisions to my progress notes there are.
3. Since the beginning of the semester I have successfully completed the goal of incorporating the
psychosocial health of my patients. For every patient that I had, I made sure to talk about their
family, or their previous career or anything to spark a conversation and build a relationship.
Although I achieved this goal, I will continue to incorporate this into my practice for the rest of
the semester as well as in my future career.
