I, LOUIE EUDELA ALVISOR, Filipino, of legal age, married, and with residence at Maramag,
Province of Bukidnon, do hereby name, constitute and appoint NOE JR. S. ELIZAGA, Filipino, of legal age,
single and with residence and postal address at 21 Rose St., Panorama Homes Subd., Buhangin, Davao
City, to be my true and lawful Attorney-in-Fact for me, and in my name, place and stead, to do any or all
of the following acts, deeds, and things, to wit:
1. To transact with the BUSINESS BUREAU OF THE DAVAO CITY GOVERNMENT regarding the
RENEWAL OF MY BUSINESS PERMIT FOR 2019, and to sign all papers in connection with said
transaction for and in my behalf;
HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full power and authority to do
and perform any and every act, thing whatsoever requisite or necessary or proper to be done in and
about the premises, as fully to all intents and purposes as I might or could do if personally present, shall
lawfully do or cause to be done by virtue of these presents with full power of substitution and
revocation, and thereby ratifying and confirming all that my said attorney shall lawfully do or cause to
be done by virtue hereof.
