Instructions: (1) Accomplish at least 4 copies of this form, and submit 2 copies to LEB with the Registrar’s Certification as
honor graduate, and the Honor Graduate Eligibility Certificate issued by the Civil Service Commission attached. Retain 1
copy for your file; (2) Pay the P300 application fee when you submit the application. For applications submitted by
mail/courier, enclose the payment with this form. Use postal money order payable to the Legal Education Board for mailed
applications, or if sent by courier, by money remittance of the courier addressed to Rovick Relleve; (3) Applications with
incomplete entries or missing attachments shall be denied; (4) Submit with your application a self-addressed stamped
envelope or prepaid pouch if you want the result transmitted to you by mail or courier; (5) Applications with incomplete
entries or missing attachments will not be accepted or denied.
