An MRI was performed on Mrs. Jaya Shukla's left knee following a fall and pain. The MRI revealed mild osteoarthritis changes including osteophytes, subchondral marrow edema, focal cartilage erosion, decreased joint space, and effusion. Examinations of the menisci and ligaments found diffuse hyperintense signals in the posterior horns of both menisci indicating mucoid degeneration or grade 1 tears. The anterior cruciate ligament showed diffuse hyperintense signaling and widening consistent with mucoid degeneration or a grade 2 tear. Sprain of the medial collateral ligament was also seen.

STUDY PROTOCOL:
FAST SPIN ECHO PD AND FAST SPIN ECHO T2W SAGGITAL IMAGES OF LEFT KNEE JOINT WERE
OBTAINED ON DEDICATED PHASED ARRAY KNEE JOINT COIL USING 1.5 TESLA HIGH GRADIENT
SYSTEM AND CORRELATED WITH T2W AXIAL IMAGES. ADDITIONAL STIR CORONAL AND T1
CORONAL IMAGES WERE ALSO OBTAINED.

Clinical History: Fall, left knee pain

FINDINGS:

Bones and joints

Mild marginal osteophytes are seen in the lower femur, upper tibia and patella. Focal marrow edema is seen in the
subchondral location of the tibial spine, more in the anterior aspect. Medial tibio-femoral joint space is mildly
decreased. Mild fluid collection is seen in the joint space.

Rest of bones are normal.

Articular Cartilage

Focal grade I-II erosion seen over the medial patellar facet.

Menisci

Medial meniscus: Diffuse PDFS hyperintense signal is seen in the posterior horn without extension into the articular
surfaces.

Lateral meniscus: PDFS hyperintense signal is seen in the posterior and anterior horns without extension into the
articular surfaces.

Ligaments

Diffuse PDFS hyperintense signal with widening of the ACL fibres in the entire course seen.

Sprain of MCL is seen.

PCL and LCL are normal

Soft tissues

Mild edema is seen in the infrapatellar hoffas fat pad. The muscles and tendons around the knee joint and
intramuscular planes are normal. The neurovascular bundles are intact.

Extensor mechanism

Patellar and quadriceps tendons are normal.

Popliteal fossa

Normal.

IMPRESSION:

Mild osteoarthritis changes in the knee joint (Osteophytes, subchondral focal marrow edema, focal
articular cartilage erosion, decreased medial tibiofemoral joint space and mild effusion)

Diffuse PDFS hyperintense signal in the posterior horn of medial meniscus without extension into
the articular surfaces - Mucoid degeneration / Grade I tear

PDFS hyperintense signal in the posterior and anterior horns of lateral meniscus without extension
into the articular surfaces - Mucoid degeneration

Diffuse PDFS hyperintense signal with widening of the ACL fibres in the entire course - Mucoid
