1. PURPOSE

The purpose of this Procedure is to ensure that where employees or others are exposed
to work situations that carry mechanical and electrical risk (electrical shock, burns, fire or
explosion), the work situation is assessed and a suitable control system is implemented
to ensure that electrical risks are removed or reduced as far as is reasonably practicable
during testing, construction, commissioning, maintenance, cleaning, inspection or repairs
to equipment, or systems.
This is to establish a safe system of control, which will prevent the unexpected release of
Electrical, Mechanical, Hydraulic, Pneumatic, & Thermal Energy.
This procedure will identify and outline the control of the energy supplies at construction
sites and related facilities where ETA has scope of works.
This document is based upon the requirements of the Kahramaa, QCS & Project
Specification.
This method statement will be applicable for both temporary and permanent power
supplies.

2. OBJECTIVES

3.1 To establish a detailed procedure to control the energization of all electrical power
within the scope of ETA’’s work of this project in a safe and organized manner.
3.2 To prevent the inadvertent operation or energization of equipment and/or systems in
order to protect all personnel.
3.3 To establish a detailed procedure to control the de-energization of electrical power
for any system or system component within the scope of work of this project in a safe
and organized manner.
3.4 To comply with regulatory standards and project management standards for working
with electricity.

3. SCOPE

This procedure establishes the minimum requirements for the prevention of any
unexpected start-up energizations or the release of any stored/ residual electrical
energy sources.
The minimum requirement of this Procedure is to ensure that an appropriate level of
safety and compliance with safety standards is the norm for all project personnel, in
order to prevent injury, pollution, and property damage while installing,
commissioning, servicing or maintaining electrical systems, equipment, machines or
system components.
This procedure describes the sequences to be followed for the energisation and de
energisation of systems or system components in a safe and organized manner by
the incorporation of the Lock Out / Tag Out Procedure.
The procedures outlined in the LOTO Procedure will apply throughout the pre-
commissioning and commissioning phases when work or test activities require that a
system or systems components be energized or isolated and de-energized as
required.
Under no circumstances will works on live electrical systems be permitted unless
there is an explicit requirement for testing or commissioning activities.

4. ELECTRICAL HAZARDS
The electrical hazards are mainly due to –
1. Electrocution or electric shock,
2. Burns,
3. Arc blasts,
4. Fire and explosion,
5. Falls

4.1 Clues that Electrical Hazards may exist –


 Tripped circuit breakers or blown fuses,
 Hot/ heat-up tools, wires, cords, connections, or junction boxes,
 Dim and flickering lights,
 Sizzles and buzzes-unusual sounds from electrical system,
 Odor of hot insulation,
 Mild tingle from contact with case or equipment,
 Ground fault Circuit Interrupter (GFCI) that shuts off a circuit,
 Worn or frayed insulation around wire or connection,
 Burn marks or discoloration on receptacle plates or plug prongs

4.2 Control Measures


The following control measures shall be in place wherever required:
 All electrical wiring / installations shall conform to the provisions and
requirements of Qatar Regulatory authority (KAHRAMAA),

 Only qualified and competent person shall be employed to handle electrical


works,
 Necessary permit to work system shall be in place to assess the risks involved
prior to start of any work,
 Prior to start of any electrical work, the electrical source shall be disconnected by
isolating the system and the ‘lock out tag out system’ shall be in place to prevent
accidental re-energizing of the circuit,
 Necessary PPE shall be provided to personnel carrying out the electrical works,
 Personnel working shall not wear rings, watches, jewellery, and other conducting
materials,
 Explosion proof boxes shall be used in areas where there is a potential for the
presence of flammable gasses or liquids to avoid explosion hazard,
 Non insulated tools shall not be used while working at electrical installations,
 Ensure that all electrical equipment are grounded properly,
 All electrical cables shall be routed through proper conduits,
 If a tripping device (ELCB, MCB) goes off, then the connection should not be
restored until the fault(s) is identified and repaired.
 Appropriate signage shall be displayed at locations where electrical equipment
are located. While carrying out the electrical works, the Supervisor concerned
shall ensure that adequate warning signs are placed at visible locations to ensure
unauthorized entry.
 Adequate illumination will be maintained in the working area.

4.3 Good practices-handling electrical equipment


Ensure the following provides minimum requirements when using equipment:
 Inspect portable cord-/ plug-connected equipment and extension cords visually
for external defects such as loose parts, deformed and missing pins, burns or
scorch marks, or damage to outer jacket or insulation and for evidence of
possible internal damage such as signs of pinching or crushing before use.
 Handle all cord / plug-connected electric equipment, flexible cord sets (extension
cords), and portable electric equipment in a manner that will not cause damage.
 Do not use the flexible electric cords connected to equipment for raising or
lowering the equipment.
 Do not fasten flexible cords with staples or otherwise hung in a fashion that could
damage the outer jacket or insulation.
 Remove the defective or damaged item from service if there is evidence of
damage that might expose an employee to injury, and do not use the item until
repaired and tested to ensure the equipment is safe.
 Check the plug-end and the receptacle whenever an attachment plug is
connected to a receptacle (including extension cords) to ensure that they are of
proper configurations and that the fit is snug.
 Ensure that an extension cord used with grounding-type equipment, contain an
equipment grounding conductor.
 Do not connect plugs and receptacles or alter it in any manner that would prevent
proper continuity of the equipment grounding conductor at the point where plugs
are attached to receptacles.
 Do not cut off the ground prong on a plug. (Additionally, those devices may not
be altered to allow the grounding pole of a plug to be inserted into slots intended
for connection to the current carrying conductors.)

 Ensure that portable electric equipment and extension cords used in highly
conductive work locations (such as areas with stagnant water) or in job locations
where employees are likely to contact water are approved for those locations.
 Do not use adapters that allow continuity of the equipment grounding connection.
 Do not use wet hands when plugging and unplugging flexible cords and cord- and
plug-connected equipment if energized equipment is involved.
 Handle energized plug and receptacle connections only with insulating gloves if
the condition of the connection could provide a conducting path to the hand. For
example, a cord connector is wet from being immersed in water.
 Secure locking-type connectors properly after connection

5. PRE-COMMISSIONING PROCEDURE
The pre-commissioning activities include visual inspection, cold testing, Hi-Pot
test, supply of 220 volts on the system, and any other activities prior
commissioning work. Work Notification will not be required in any T&C works that
will not use any external power supply i.e. visual inspection, readiness check, and
cold testing. T&C team will notify one day in advance the ETA HSE department
by submitting a duly filled Work Notification Form for all T&C activities that
requires use of power supply (temporary or permanent). Pre-commissioning
activity can only proceed after complying with all safety requirements of ETA &
SMJV and approval of work notification. T&C team must observe PTW system
and Lock-out and Tag-out procedure where applicable.

6. LOCK OUT AND TAG OUT PROCEDURE

6.1 DEFINITIONS AND ACRONYMS

a. Lockout / Tagout (LOTO): The operation of de-energizing and mechanically


isolating a piece of equipment by placing the energy isolating devices in the
positions necessary to contain or obstruct the potential energy of the source.
The installation of locks, multiple lock hasps, tags or chains on the energy
isolating devices, to keep them in the desired positions so that the equipment
can be safely worked on.

b. Isolating device is a mechanical means of physically preventing any


transmission or release of energy. This may take the form of the manual
operation of a switch, a circuit breaker, a line valve, block valve or any similar
device, which ensures continued energy isolation. It does not include on/off
or push button type devices.

c. Lock is a padlock (and key) which will be used to hold an energy switching
device in the de-energized position. They shall be the same type used within
the project construction, pre-commissioning and commissioning. There shall
be only one (1) key per lock and no master keys or combination locks will be
allowed. The key to the lock when being used to lock out a device shall
remain with the LOTO Coordinator.

d. Lockout is the placement of a locking device on an energy-isolating device,


ensuring that the energy isolating device and the equipment being controlled
cannot be operated until the lockout device is removed.

e. Lockout device is the device, which is used to render and maintain


components or systems in the safe mode and will normally be in the form of a
padlock, chain, or other mechanical locks.

f. Key box is a lockable box used to hold the keys for the locks employed on
equipment or machinery, controlled by the LOTO Coordinator located in the
Energized Area/ Room and is locked at all times.

g. Tagout is the placement of a tag on an energy-isolating device, to indicate


that the equipment being controlled (listed on Tag) shall not be operated until
the lockout device is removed.

h. Tagout device refers to the actual tag or warning sign, which is securely
fastened to an isolating device to indicate that the equipment or system is
under controlled isolation and cannot be operated.

i. Tags shall be standardized in print, and format and have the wording DO
NOT OPERATE printed on the face of the tag. The attachment of the tag
shall be non-reusable type, attached by hand, self -locking nylon cable tie.
The tag shall be used only (1) time and then be destroyed. Tags shall be filled
out using permanent marker, and must be legible. Tags will be signed by the
issuing authority with the Authorized Employee’s number, dated and attached
to each lock. No device will be operated with a danger tag / lockout tag on it
under any circumstances until the complete LOTO removal procedure has
been completed and all tags removed.

j. Testing –Refers to conducting functional equipment tests based on the


manufacturer specifications and recommendations and includes the
recording, reviewing and documentation of results for future performance
comparisons.

k. PPE – Personal Protective Equipment


Including but not limited to the following;
 Hard Hat
 Safety Shoes
 Overalls
 Hi Visibility Vest
 Gloves
 Safety Glasses
 Safety Harness
 Electrically Insulated Rubber Gloves
 Electrically insulated ladders
 Rubber Floor mats for all Main Panels & Transformers

6.2 RESPONSIBILITIES
Nominated duty holders / authorized persons to carry out LOTO (Lockout Tag out) &
Isolation activities are identified as given below:

a. T & C Manager
He has the overall responsibility to ensure full compliance with requirements
described in this procedure on site.
He shall ensure the following
 All of his personnel attend the training provided
 All employees comply and actively participate with the requirements of this
procedure
 He demonstrate his commitment to safety, health and the environment
 He will immediately correct any uncontrolled hazards identified
 He will verify the implementation of this procedure
 He will instigate a prompt correction of any possible deviation from procedure.
 He will provide personnel protective equipment, facilities, tools and equipment, and
other adequate resources to effectively implement, administer and enforce this
procedure
 He will confirm the appointment and authorization (in writing) of competent persons
including the Lock Out/ Tag Out (LOTO) Coordinator.
 He will at all times be aware of any systems being energized or de-energised

b. HSE Officer is responsible to co-ordinate with the LOTO Co-coordinator for,


inspecting tagging and locking out devices.
He shall carry out the following duties;
 Monitoring all personnel to ensure adherence to this procedure.
 Advise & assist as necessary to ensure a safe environment at the work location
 Assist the supervisor to plan and coordinate the work to effectively implement the
requirement of this procedure
 Provide assistance and training for the team as requested.
 Ensure that regular tool box talks are conducted for all activities.
 He will regularly co-ordinate with the T&C In-charge, PM, CM, and Main Contractor
HSE Officer with regards to T&C activities.
 HSE officer will conduct periodic reviews of the implementation of the this Plan.
 The HSE Officer will ensure that the specific Method Statement & Risk Assessment
for energizing the particular piece of equipment has been submitted and approved.

c. LOTO Coordinator is the lead person responsible for applying and managing the
lock out tag out procedures on site.
He is a competent person who is responsible for
 Generating all permits and issuing all locks & keys
 Maintaining the key box and the LOTO log book
 Controlling the key to the key box
 Monitoring the validity of all open permits
 Ensuring permits are closed
 Ensure that no panel will be energized without the correct test sheets in place.
 Ensure that all pre testing (cold testing) criteria have been fulfilled.
 He will maintain the schematic & layout drawings in the LOTO office that indicate
the electrical systems being energized or de-energized.

d. LOTO Issuing Authority:


Is a competent person who issues and signs LOTO permits for the machine, piece
of equipment or system to be locked out.
He shall;
 Review all requests for energisation or de-energisation
 Authorize any energisation or de-energisation in the systems under his control, if
safe to do so.
 Check the final condition of isolation on site for effectiveness and completeness.
 Sign all work permits & work notification.
 He shall sign and close the LOTO permit to confirm that work was completed and
the isolation devices have been removed and are in safe mode.

e. Receiving Authority:
Is the Work In-charge (Supervisor/ Engineer) who is involved in the task and is
responsible for executing the task in a safe and organised manner.
 He will ensure the instructions laid out in the Electrical Safety Plan are followed
 He will monitor the work at the site at all times to ensure that the Electrical Safety Plan
is being adhered to
 He shall never remove, bypass, ignore or defeat the lock out / tag out system
 He will conduct a suitable Tool Box Talk with his crew prior to work commencing on a
particular task
 He will be responsible for the erection of warning signs and barriers as required

 Ensure that no circuit will be energized without the correct test sheets in place.
 Ensure that all pre testing (cold testing) criteria have been fulfilled.

f. Authorized Employee/s:
Any employee who performs any work, or participates in a hazardous energy lockout
/ tag out. He will be appointed by the TNC Manager to identify the system or system
components to be locked out and to inform the LOTO Coordinator.
 He is responsible for controlling and maintaining accountability of tags (Tag out
devices)
 He will coordinate construction isolation activities
 He will advise other entities and trades that may be affected by or are in the vicinity of
LOTO activities; All the people will be informed thru toolbox talk, information letters
and open forums about the task before starting the testing. Area will be barricaded
and warning signs posted. Watch man will be available to stop unauthorized entry. All
the details about the task will be discussed with main contractor prior to start the
testing. Testing only be started after receives confirmation from main contractor permit
to work.
 He will assist the supervisor with the erection of warning signs and barriers as
required
 He will maintain a log for controlling and tracking tags and the making or receiving of
requests for tagging

g. Employees – All employees involved in this project must know, understand and
comply with the requirements of the LOTO procedure (including all trades, staff and
subcontractors).
 They shall attend the relevant training courses and tool box talks as scheduled and be
aware of potential hazards.
 They shall at all times wear the appropriate PPE and immediately report any
deficiencies or problems to their supervisor.
 They shall carry out their task in a safe manner at all times with respect to themselves
and their colleagues.
 They shall never remove, bypass, ignore or defeat the lock out / tag out system.

Note:
 TNC team including suppliers will use Orange Helmet while Energized Area/ Room
watchman will use White Helmet.
 Lock & Key box will be kept in the Energized Area/ Room and will be at all times
protected from theft and abuse. Only one key will be kept in the Key Box and the
spare key will be kept in TNC Office. The key of the lock box (key box) will be in the
possession of the authorized person and the watchman in the area.
 HV room key (Master Key) will be kept by Sr. TNC Engr - Electrical. Emergency Key
(Spare HV room key) will be kept by Area ETA Safety Officer. It will be used only for
emergency and HV room inspection.
 Free standing information board will be kept in Energized Room/ Area. Key box,
permit, and ID Card will be kept in this board.
 All authorized person will be given ID card based on recommendation of TNC
Manager or Sr. TNC Engineer – Electrical. The ID card will be placed on the board
where Key Box will be kept whenever the authorized person will padlock any
system.
 LOTO log book will be kept under the control of LOTO Coordinator. This will cover
the permit to work system, section for testing and access
 The padlock key will not be issued at any circumstance to ny personnel unless
signed by authorized TNC Engineer or Project Manager.
 Initially all breakers in all main panels and feeders will be locked “off”. Stage by
stage energization will be carried out with proper procedures & permits.
 When entering energized room/ area:
 PTW system will be required on all work in energized room / area. PTW will be
issued by ETA Sr. Electrical Engineer, Sr. TNC Engineer – Electrical and area
HSE Officer, and PTW is to be approved by concerned Department head.
 The ID Cards will be placed on the information board when locking any system
for de-energization and key will be kept in the key box.
 After completion of the task, pre-test is done, all is in order, and confirming that
the area is clear of people, the system will be energized by Authorized Person
by removing the locks. Keys and padlock will be returned to the key box and the
ID of the authorized person will be removed from information board.

6.3 ENERGISATION PROCEDURES

a. Energisation Pre-checks

All testing and commission activities will be carried out through a work notification
duly authorized by the T&C Manager also called the LOTO Issuing Authority. The
work notification will have the concurrence from all competent authorities as deemed
necessary by the issuing authority.
The work notification will be received by the competent person in charge of the work
called the receiving authority.
A permit to work in live areas will be issued by the safety advisor after verifying the
work notification and associated controls mentioned in the work notification.
 The competent person responsible for the work (called the work in-charge) will
ensure that all supplies are locked out in accordance with the LOTO procedure prior
to commencing the job.
 The Authorised Employee shall fully identify the system or component to be
energised in writing (and highlighted on a drawing) and specify all points that are
locked out and tagged out and which locks need to be removed.
 The Supervisor & LOTO Coordinator will ensure that all pre testing (cold testing)
criteria have been fulfilled.
 The Supervisor & LOTO Coordinator will ensure that no circuit will be energized
without the correct test sheets in place.
 The Authorised Employee will ensure that all personnel (all trades) in adjacent areas
are made aware of the imminent activities and associated consequences, or are
excluded from areas of potential danger. This will be done thru a proper
communication, hard barrier and proper sign board will be provided. TBT arranged to
all those who working in the vicinity about the risk prior to testing. All activity other
than testing will be kept in safe distance.

 The Supervisor will ensure that all members of the team have been trained and are
aware of the requirements of the Electrical Safety Plan and are aware of their own
specific roles and responsibilities.
 The HSE Officer will ensure that a toolbox talk has been conducted and that the
relevant SPA and Risk Assessment have been carried out properly.
 The Supervisor will observe the safety measures, procedures and any
manufacturer’s Instructions with respect to the particular piece of equipment to be
energized.
 The HSE Officer will ensure that the specific Method Statement for energizing the
particular piece of equipment has already been submitted and approved.
 The HSE Officer will ensure that all requirements of this specific Method Statement
have been followed with the risk control measures in place.
 The Authorized Employee will ensure all test equipment has current Calibration
Certificates

b. Energisation

 As stated above, all supplies will be locked out in accordance with the LOTO
procedure prior to commencing.
 Signage (Danger High Voltage, Entry By Permit Only, NO Entry – Authorized Person
Only, etc)) and hard barriers will be erected in and around the affected area informing
the activities to be undertaken and to prevent uninformed people from entering the
adjacent areas.
 On instruction from his Supervisor, the Authorized Employee will advise the LOTO
coordinator of the permit requirements and hand over the necessary information and
drawings identifying the scope of the energisation.
 The LOTO Coordinator will check and prepare the documentation and submit the
necessary Permit to the LOTO Issuing Authority.
 The LOTO Issuing Authority will check the documentation and inspect the site with
the LOTO Coordinator and the HSE adviser.
 The LOTO Issuing Authority will then approve the permit and issue it to the LOTO
Coordinator.
 The LOTO Coordinator will then source the appropriate keys for the particular locks
and review the installation with the Supervisor and HSE Adviser, removing the locks
once satisfied all is in order.
 The system is now energized and the commissioning activities may commence.
 The LOTO Coordinator then returns the lock and key to the key box and records the
energisation in the LOTO logbook and on the LOTO schematic and layout drawings
located in the LOTO office.
 All operators those who involved in the task will be briefed about the risk & control
measures.

6.4 DE-ENERGISATION PROCEDURES

a. De Energisation Prechecks

 In this instance, the system in question is assumed to be ‘live’.


 Fully identify the system or component to be de-energised in writing (and highlighted
on a drawing) and specify all points that are required to be locked out and tagged
out.
 The Supervisor will advise the LOTO Coordinator what system or system component
is to be de-energised and for what reason.
 The LOTO Coordinator reviews the relevant drawings and documentation to
determine if any other systems would be affected by the proposed de-energisation.

 If any other systems would be affected by the proposed de-energisation, the LOTO
Coordinator will request clearance to proceed from the system owners.
 The HSE Officer is consulted to review the consequences and required precautions
of the proposed de-energisation. (Signage, Emergency Lighting, Accessibility etc.)
 The Authorised Employee will ensure that all personnel (all trades) in adjacent areas
are made aware of the imminent activities and associated consequences, or are
excluded from areas of potential danger. This will be done thru a proper
communication, hard barrier and proper sign board will be provided. TBT arranged to
all those who working in the vicinity about the risk prior to testing. All activity other
than testing will be kept in safe distance
 The Supervisor will ensure that all members of the team have been trained and are
aware of the requirements of the Electrical Safety Plan and are aware of their own
specific roles and responsibilities.
 The HSE Adviser will ensure that a toolbox talk has been conducted and that the
relevant SPA and Risk Assessment have been carried out properly.

 The Supervisor will observe the safety measures, procedures and any
manufacturer’s Instructions with respect to the particular piece of equipment to be
energized.
 The Authorised Employee will ensure all test equipment has current Calibration
Certificates that are available with the Testing & Commissioning Department.

b. De-Energisation

 As stated above, in this instance, the system in question is assumed to be ‘live’.


 Signage (Mains Voltage De-Energisation) and hard barriers will be erected in and
around the affected area informing the activities to be undertaken and to prevent
uninformed people from entering the adjacent areas.
 On instruction from his Supervisor, the Authorized Employee will advise the LOTO
coordinator of the permit requirements and hand over the necessary information and
drawings identifying the scope of the de-energisation.
 The LOTO Coordinator will check and prepare the documentation and submit the
necessary permit to the LOTO Issuing Authority.
 The permit will include clearance signatures from System Owners of any other
affected systems. (if required)
 The LOTO Issuing Authority will check the documentation and inspect the site with
the LOTO Coordinator and the HSE Officer.
 The LOTO Issuing Authority will then approve the permit and issue it to the LOTO
Coordinator.
 The LOTO Coordinator will then obtain the required number of locks and keys from
the Lockbox and review the installation with the Supervisor and HSE Officer,
installing the locks once satisfied all is in order.
 The Authorised Employee installs all tags at the required locked out locations. Tags
will be signed with the Authorised Employees number, dated and attached to each
lock with a self locking cable tie.
 The Authorised Employee will check the Locked out System voltage and
demonstrate to the Supervisor, LOTO Coordinator and HSE Officer that the system is
de-energised. (zero voltage reading on the voltmeter)
 The LOTO Coordinator will then return the key to the Lockbox and record the de-
energisation in the LOTO logbook and on the LOTO Schematic and Layout drawings
located in the LOTO office.
 A copy of the Permit will be clearly posted at the location of the de-energisation.

7. EMERGENCY PROCEDURE
 Main Contractor’s emergency plan will be adopted throughout this project
 Rescue plan (location of the isolating devices, how it will be operated, escape
route, etc) and procedure to be developed for each substation.
 Communication media to be established and provided (Radios or mobile)
 Electrical Emergency Kit and fire extinguisher (CO2 to be made available at site
 Emergency contact numbers to be displayed at work area.

EMERGENCY ACTION PLAN

 IN CASE OF ANY ACCIDENT/INCIDENT


 Inform your Supervisor who will summon the first aider immediately if
required.
 Inform ETA HSE Manager who will notify Main Contractor’s HSE
Manager & ETA Sr. Project Manager if required.
 Notify Emergency Services if required.

a. Electric Shock Emergency

 In the event of electric shock, always isolate the supply


 Ensure the re-energization
 Do not touch the electrocuted person with bare hands. Use electrical
emergency kit
 Isolate the injured person and the incident area if possible
 Don’t panic. Do not put yourself at risk
 Get help. Inform your Supervisor/ First Aider/ Safety Officer at the earliest.
Give the exact location and nature of the incident.
 If you are a trained first aider, check the ABC and conduct CPR if
required.

b. Other Emergencies like Electrical Fire, Explosion, Arcing, etc
 Don’t panic
 Don’t attempt to take your tools out
 Immediately isolate the electrical supply from the main incomer or ask
your team leader or supervisor by walkie talkie or mobile to isolate the
system at the earliest.
 Don’t put yourself when trying to save anybody
 Inform your Supervisor/ Safety Officer at the earliest. Give the exact
location and nature of the incident.

8. TRAINING
 Only competent person (education, training, and experience) will be assign in key
positions for testing and commissioning activities. They should be trained by
recognized 3rd Party training institute on LOTO procedure. All other employees
involved in testing and commissioning should be trained internally on LOTO
procedure.

 No person shall be engaged in any work activity where technical knowledge and
experience is required unless he possesses such knowledge and experience, and
is under such degree of supervision to prevent incident or accident having regard
for the nature of the work.

 The control of hazardous Energy training will be brought to the attention of ETA T &
C personnel and to their subcontractors at induction. A scheduled class will be
conducted for those employees identified as lead personnel with respect to the
Electrical Safety Plan.
