October 1, 2018  To attend PCOM
Sevillas - - The meeting started around 9 in the morning, opened by
Restaurant, Brgy. A morning prayer, National Anthem, Roll call per m
Domoit Lucena municipality, new staff, presentation of the agenda and
City reading of the previous minutes.
- -For BDM Ma’am Leonor Batchain discuss the validation of - Submission of Code 15-
code 15, Minor Granteeand the IPC target C14 will be on Oct. 5, 2018
- For GRS Sir Jordan discuss thing related with - Request for Retro may be
grievances and Retro Payment pass every PCOM
- For UNIFAST Sir Arnel Buefano discuss some update - Students may follow-up in
regarding how to follow up with the school of the student their own school to have an
and data per municipality update regarding the Uni-Fast
- For UFMS they discuss regarding the turn-out of the -Submission of NMA will be

Month of Aug. And 10 absent for the month of Sept.
- For Brgy. Bagong Silang there were only 1absent last Aug
- For Brgy. Bolo there were 6 absent for Aug. And 6 ansemt
for Sept.
October 3, 2018  Conduct of YDS
Brgy. Magsaysay - conducted YDS session 4 for Grade 8 student with 40

Month of Aug. And 14 absent for the month of Sept.
- For Brgy. Ilayang Cambuga there were 16 absent for the

- For Brgy. Latangan there were 13 absent for Aug. And 12
ansemt for Sept.

Mulanay, Quezon -Assisted Walk in Beneficiaries regarding their inquiries

-CSO Meeting
- Printing of the FDS attendance sheet for Brgy. Mabini,
attendance of the GST and Printing and cutting of
certificates, hands and name tag to be used.
October 9, 2018  Conducted Family Development Session for PG 1-6 The next FDS will be on
Brgy. Mabini - Conducted FDS Topic on CSAP: where there area 31 Nov. 21, 2018
Mulanay, Quezon present for PG 1, 21 present for PG 2, 21 present for PG 3, Attendance of the
20 present for PG 4, 25 present for PG 5 and 18 present for spouse for GST
PG 6. tomorrow is a must
October 10, 2018  Gender Sensitivity Training for Male Spouses
Brgy. Mabini - Conducted GST at Brgy. Mabini, where the group was
Mulanay, Quezon devided into 3, where ML Leony and ML Rudy conducted
for PG 1-2 who have 43 participants, ML Jen and ML Joy
conducted for PG 3-4 where they have 32 participants and
ML Geserie and ML Lhen conducted for PG 5-6 where they
have 30 participants. Proper discussion and activities were
done.
October 11, 2018  Office Work The next yds will be on Nov.
Mulanay, -Assisted Walk in Beneficiaries regarding their inquiries 9, 2018
Quezon and other concern
- Doing of Calendar, TRF and preparing of the raw data for
the Case Conference
- Preparing the September attendance and FDS
Documentation.
 Conducted Youth Development Session
-Conducted YDS Session 4 and 5 for Grade 7 student with
27 participants.
Proper discussion and activities were done.
October 12, 2018  Cluster Case Conference - there must be 2 Csae
th
St. Jude Coop - Have attended the 4 Cluster Case Conference where Study submitted per
and Hotel SWO-III Gerald first gave quiz, where ML Geserie and ML/month.
Tayabas, Quezon MIlbren got the highest score and the lowest score will be
the one who will present their case study. Followed by the
review on the 7 principles, disussion of different theories
and the Case Study Workshop.
- Discussion of how to compose the proper Problem
