Identifying Data and Reason for Referral

John is a 16-year-old Asian-American adolescent who was recommended for psychological

assessment to establish his mental and emotional condition. John has been performing poorly in

his academic work in spite of having a private instructor and securing accommodation in school.

The factors that have influenced his performance include poor attendance of lessons, learning

challenges and family problems.

Sources of Information

The confidential information was obtained from John’s mother, the nurse, the psychologist,

and medical data. The data was collected from interviews, medical information, assessment

scales, and development records. The sources of the information were reliable.

The information on his current condition of education and conduct was collected through

observation during the analysis and tests on his psychological as well as neuropsychological

condition. The validity of John’s performance was accurate because of his active participation in

the tests.

Background Information

Current Concert

John has been struggling with his academic work and social conduct. He has a complicated

relationship with his parents. One of the main concerns is to sustain the morale of John in school

work. The major issue in his case is his addiction to marijuana.

Medical and Development History

Pregnancy and Birth History

John’s mother claimed that she was thirty and the father was thirty-one years when she gave

birth. She did not have complications during the pregnancy and delivery. John was born through

Caesarean section. John’s birth weight was normal but he encountered jaundice after delivery.

Developmental History

John had sleeping disorders after birth because he could not sleep more than three hours

without getting interruptions. His appetite was poor. John’s developmental milestones were

normal including speech and gross motor improvement. He has a poor coordination, particularly

in writing. The development of bladder control was late and he peeped uncontrollably at night on

his bed. He encountered several accidents in his childhood which resulted in breaking his bones.

Medical History

He contracted usual illnesses during his childhood which include chickenpox, ear diseases

along with throat illnesses.

Family and Social Background

Grace Brown is John’s biological mother and she is a pilot. She claimed that she no

knowledge or attention challenges. David Brown is John’s biological father. John’s mother

claimed that her husband is not a full-time employee. David worked at the grocery and stores

when they were married. He was violent to Grace and John. Mr. and Mrs. Brown are separated

and John has a step-sister. Mary is hyperactive and has poor writing skills. John’s mother

attested that the family has a history of depression.

Summary of Past Psychological Evaluations

John has undergone various examinations to establish the causes of his learning and

behavioral challenges. In 2008, his psychologist carried out a neuropsychological examination of

John. It was found that John had mixed dominance in his hands and left dominance in his eyes.

John prefers to use his left hand than the right one. His mental capacity had a score of below

average with a difference in communication and attainment skills. John has challenges in the

visual motor, visual sequence, and processing activity. John’s verbal memory was higher than his

visual memory. He has challenges in spelling and handwriting but his speech is efficient. The

psychological examination indicated that John had anxiety challenges.

The follow-up analysis and school proposal included the main-stream choice to be adopted. It

included the design and proposition of his daily routines. It was the responsibility of John’s

family to implement the recommendation in his chores.

In 2011, a psychological analysis was carried out with an aim of further examination and

medication plan. The psychologist has suspected that John could have a depressive disorder

which needed treatment. It was found that there was a high score in depression and anxiety. The

results indicated that John had challenges with concentration. The achievement examination in

2013 illustrated that the milestones were in line with his abilities for the first time.

Another analysis carried in 2016 using WIAT and it was discovered that John score on

mental capacity was average. His verbal activities had a higher performance as compared to the

non-verbal tasks. John experienced delays in mathematics as well as writing.

Behavior Observations

Home: John’s mother claimed that he likes singing and skateboarding. He prefers to be given

directions and cautious in making mistakes. He has a regular class with a private instructor.

School: John’s teacher indicated that he has low motivation in school work. John has

challenges in reading and writing. He is good in language and speech.
