MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered into in Tuguegarao City, Cagayan,
by and between the Cagayan State University (CSU), represented by Dr. URDUJAH A. TEJADA,
CSU President, hereinafter referred to as FIRST PARTY, and the
__________________________________________________________, represented by,
___________________________________________ hereinafter referred to as SECOND PARTY,

WITNESSETH;

WHEREAS, the Cagayan State University (FIRST PARTY) is a higher educational institution
duly organized and existing under the laws of the Republic of the Philippines, with main office
address located at Caritan Centro, Tuguegarao City, Cagayan offering various engineering
courses;

WHEREAS, CSU, has requested for an On-The-Job Training (OJT) for Electronics
Engineering student;

WHEREAS, _______________________________________________ (SECOND PARTY)


has granted the request for the On-The-Job Training (OJT) of the students;

WHEREAS, the FIRST PARTY and the SECOND PARTY recognize the need to train future
engineers to be the catalysts of industrial development of Cagayan Valley Region;

WHEREAS, the PARTIES recognize that strong partnership with other


agencies/companies can significantly improve their effectiveness in carrying out their vision,
mission and objectives;

NOW, THEREFORE, for and in consideration of these above premises and other mutual
covenant hereinafter set forth, the parties do hereby stipulate the following:

That CSU SHALL:


1. Send students who shall undergo On-The-Job Training;
2. Require the OJT students submit an On-The-Job Training Agreement and Non-
Liability Waiver to both parties;
3. Direct the OJT students to abide to all the link agency/company’s work rules and
regulations;
4. Require the OJT students sign in and sign out daily time record in accordance to the
link agency/company rules;
5. Not ask or receive any allowance, remuneration, and benefit from the link agency;
6. Let the OJT student render a total of 240 OJT hours;
7. Require to submit comprehensive report of the OJT activities.

That the _________________________________________________________, SHALL:

1. Provide an orientation session to the students concerning the On-The-Job Training;


2. Provide opportunities for students to be exposed to electronics engineering
practices;
3. Inform CSU immediately on problem relating to any student trainees;
4. Evaluate the performance of the student using the Evaluation Sheet provided by the
CSU at the end of the training;
5. Ensure that safety procedures are followed by the OJT student on site.

IN WITNESS WHEREOF, the parties have set their hands and cause the signature of their
respective representatives to be affixed on this ______ day of __________, 20___ at
Tuguegarao City, Cagayan, Philippines.
