    Information for entry into Shanghai (China) to the Master

    Dear Masters

    We welcome you and your good vessel to call upon port of Shanghai.
    Following port information may be useful for your arrival, also will mail some
    blanks to you via port of Hong Kong. By the way, please fax the "Ships
    particulars" and Certificate of "Documents of Compliance of construction and
    equipment with the special requirements for ship arriving with "dangerous
    goods" to us at your convenience.

    All Masters are required to send ship's message to our ship agent - UNISCO
    72, 48 and 24 hrs prior to her expected arrival at port of Shanghai or on her
    departure from the last port of call. If the voyage takes less than 24 hours
    alternation should be advised promptly if there is any change occurred within
    24 hrs before arrival. The ship's agent (China ports/Unisco) will pass the
    message to the "port authority", "harbour master", pilot and parties concerned
    when receiving.

    For port of Shanghai the vessel's information should be including following:

    72hrs before sent vessel's ETA CHANGJIANKOU anchorage (please refer to


    self submitted charts) with arrival draft F/A and within follows information.

    1. Vessel' register certificate number, place and date of issue and voyage
    number.
    2. Cast port of call and loading ports with cargo name and quantity
    including through cargo.
    3. Type and quantity of arms and ammunitions if any.
    4. Any animals carried.
    5. When/Where and number of sanitary certificate issued in Chinese port if
    available.
    6. When/Where deratting / deratting exemption certificate issued.
    7. Where and when taken the provision and fresh water.
    8. Number of crew and pax if any with health state.
    9. Ports called last 10 days before arriving Shanghai.
    10. Ship's high from keel to highest point and air draft on vessel's arrive and
    departure.
    11. Crew list showing crew name, nationality, sex/rank date /place of berth
    and number of seaman's passport.
    12. If required to pump out ballast water at port (stating where taken) M/E
    maintenance and painting ship's hull etc. (if vessel requires to discharge
    slop at Sha, please give Captain Lee of HL Shanghai office 4 day notice.
    It is free of charge).
    When vessel arrives port of Shanghai the following documents should be
    prepared.

    4. Ship's Store Declaration 3 "
    5. Crew' Effects Declaration 2 "
    6. Maritime Declaration of Health 1 "
    7. Report on Ship's Particulars 1 "
    8. Health Declaration from cntr for exit/entry ship 1 "

    For out bound

    1. General Declaration 5 "


    2. Cargo Declaration 4 "
    3. Crew List 3 " (if change 6
