_____________, Philippines, upon oath in accordance with law, do hereby
depose and say, THAT:

1. I am the same person whose personal circumstances are described


in the preceding paragraph;

2. I am the mother of _________________ who is currently detained in


Lantun City Jail at Lagao, General Santos City;

3. I certify that ________________ is an indigent with low income, as


per record issued in the Office of the Punong Barangay
of________________________.

4. I am executing this affidavit to attest to the truth of the foregoing


facts and for whatever legal purpose this may serve.
